摘 要:目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者缺氧诱导因子-1α(HIF-1α)与血清肺泡表面活性蛋白-A(SP-A)、肺泡表面活性蛋白-D(SP-D)水平的影响及意义。方法选择2016年8月至2017年2月贵州省人民医院门诊及住院行睡眠监测患者83例,分为对照组20例、轻度OSAHS组20例、中度OSAHS组23例、重度OSAHS组20例。检测HIF-1α与核因子(NF)-κB mRNA水平及蛋白表达水平,血清肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6、SP-A和SP-D水平。结果重度OSAHS组HIF-1αmRNA、NF-κB mRNA水平及蛋白表达水平,TNF-α、IL-6水平明显高于对照组(P<0.05);中、重度OSAHS组SP-A、SPD水平明显高于对照组(P<0.05);血清SP-A水平与HIF-1αmRNA相对表达水平呈正相关(P<0.05)。结论缺氧可激活HIF-1α信号通路,释放NF-κB、TNF-α、IL-6等炎症因子,并引起SP-A、SP-D水平升高。Objective To investigate the effect and significance of serum hypoxia inducible factor-1α(HIF-1α)and alveolar surfactant protein A(SP-A)and alveolar surfactant protein D(SP-D)levels in the patients with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods Eighty-three patients undergoing the sleep monitoring in the outpatient and inpatient departments of the Guizhou Provincial People′s Hospital from August 2016 to February 2017 were selected and divided into the control group(20 cases),mild OSAHS group(20 cases),moderate OSAHS group(23 cases)and severe OSAHS group(20 cases)according to the OSAHS diagnosis and treatment guideline.The levels of HIF-1α,TNF-α,NF-κB mRNA and protein were measured by adopting real time PCR and Western blot.The levels of TNF-α,IL-6,SP-A and SP-D were detected by using ELISA.Results The expression levels of HIF-1αmRNA,NF-κB mRNA and protein in the severe OSAHS group were significantly higher than those in the control group(P〈0.05).The levels of TNF-αand IL-6 in the severe OSAHS group were significantly higher than those in the control group.The levels of SP-A and SP-D in the moderate and severe OSAHS groups were significantly higher than those in the control group(P〈0.05).The serum SP-A level was positively correlated with HIF-1α mRNA related expression level(P〈0.05).Conclusion Hypoxia can activate HIF-1αsignaling pathway,releases the inflammatory factors of NF-κB,TNF-α,IL-6,etc.and causes the increase of SP-A and SP-D level in OSAHS patients.
