加味补阳还五汤联合速碧林预防人工全髋关节置换术后下肢深静脉血栓形成作用研究

摘　　要：目的：探讨加味补阳还五汤联合低分子肝素钙（速碧林）注射剂在预防人工全髋关节置换术后下肢深静脉血栓形成中的作用。方法：选择2016年2月—2017年2月收治的94例需行人工全髋关节置换术患者,将其随机分为观察组（47例）和对照组（47例）。对照组采取常规的治疗干预＋速碧林,观察组在对照组的基础上给予加味补阳还五汤,统计两组患者拔除引流管量、血流速度、下肢深静脉血栓发生率,比较两组患者治疗前后凝血酶原、部分凝血酶原时间、血小板计数的变化及患者住院时间及下床时间。结果：治疗后观察组患者术后引流量（术后48 h）、下肢深静脉血栓形成率（术后7 d）均低于对照组,血流速度高于对照组,凝血酶原时间、部分凝血酶原时间长于对照组,住院时间及下床时间短于对照组,差异均有统计学意义（P〈0.05）。结论：加味补阳还五汤联合速碧林针剂可降低人工全髋关节置换术后下肢深静脉血栓形成的发生率,效果显著且安全,值得推广。Objective：To investigate the role of the Modified Buyang Huanwu Decoction combined with Subilin acupuncture on the prevention of deep vein thrombosis in lower extremity after artificial total hip arthroplasty. Methods：A total of 94 patients with total hip arthroplasty in our hospital from February 2016 to February 2017 were randomly divided into observation group（47 cases）and control group（47 cases）. In the control group,the routine treatment intervention＋ Subilin was performed and the observation group was given the Modified Buyang Huanwu Decoction on the basis of the control group,and the drainage tube was collected from 48 h after surgery. The blood flow rate 7 days after surgery and the blood flow were detected. The changes of thrombin,partial thrombin time,the changes of platelet count and the time of hospitalization and the time of bed were compared between the two groups. Results：The observation group of patients after treatment for the flow rate（after treatment 48 h）,rate of lower extremity deep vein thrombosis（after treatment 7 d）were lower than those in the control group,the blood flow velocity was higher than that in the control group,the prothrombin time,partial prothrombin time was longer than that in the control group,hospital stay and bed time was shorter than that in the control group,the difference had statistical significances（P〈0.05）. Conclusion：The combination of Modified Buyang Huanwu Decoction and Subilin acupuncture can reduce the incidence of deep vein thrombosis in lower extremity after total hip arthroplasty,and it is effective and safe and worth promoting.
