活血生肌汤联合干细胞移植干预糖尿病足作用的临床研究

摘　　要：目的:探讨活血生肌汤联合自体骨髓血干细胞移植治疗糖尿病足的临床疗效。方法:将60例糖尿病足分为常规内科治疗组(常规治疗组)、常规内科治疗组+干细胞移植(干细胞移植组)、干细胞移植+常规内科治疗+活血生肌汤联合治疗组(活血生肌汤组),治疗3个月后对各指标疗效进行评估。结果:常规治疗组治疗前后各项指标比较差异无统计学意义;治疗3个月后血糖、血脂、静息疼、足冷感觉、间歇跛行、足趾皮温、踝肱指数、血压比较,干细胞移植组、活血生肌汤组较常规治疗组比较差异有统计学意义,活血生肌汤组较干细胞移植组比较差异有统计学意义);治疗3个月后左侧患足溃疡面积/创面肉芽组织中毛细血管数量、愈合率比较,干细胞移植组、活血生肌汤组较常规治疗组差异有统计学意义,活血生肌汤组较干细胞移植组差异有统计学意义;治疗3个月后血管显影比较,常规治疗组左膝下胫后动脉阻塞,干细胞移植组左胫后动脉畅通,活血生肌汤组血流直达左足部;治疗3个月后干细胞移植组、活血生肌汤组左下肢腘动脉管径、足背动脉管径及血流速度、左下肢正中、腓神经传导速度的效果优于常规治疗组,活血生肌汤组优于干细胞移植组。结论:活血生肌汤能明显提高自体骨髓干细胞移植治疗糖尿病足的疗效。Objective： To investigate the clinical efficacy of Huoxue Shengji Decoction combined with autologous bone marrow stem cell transplantation（ABMSCT） in the treatment of diabetic foot. Methods： Sixty diabetic patients were divided into three groups： Conventional Medical Treatment Group（Conventional Treatment Group）, Conventional Medical Treatment Group ＋ Stem Cell Transplantation（Stem Cell Transplantation Group）,and Stem Cell Transplantation ＋Conventional Medical Treatment ＋ Huoxue Shengji decoction therapy（Huoxue Shengji Decoction Group）. After 3-month treatment,the curative effect of each index was evaluated. Results：（1） There is no difference before and after the treatment in Conventional Treatment Group.（2） After 3-month treatment,the comparative results of blood glucose,blood lipids,rest pain,cold feeling,intermittent claudication,toe skin temperature,ankle brachial index,and blood pressure are as follows： the curative effect of Stem Cell Transplantation Group and Huoxue Shengji Decoction Group is better than that of Conventional Treatment Group,and there is a significant difference; the curative effect of Huoxue Shengji Decoction Group is better than that of Stem Cell Transplantation Group,and the difference is statistically significant.（3） After 3-month treatment,the comparative results of the left foot ulcer area/wound granulation tissue capillary number and healing rate are as follows： the curative effect of Stem Cell Transplantation Group and Huoxue Shengji Decoction Group is better than that of Conventional Treatment Group,and there is a significant difference; the curative effect of Huoxue Shengji Decoction Group is better than that of Stem Cell Transplantation Group,and the difference is statistically significant.（4） After 3-month treatment,the comparative results of vascular imaging are as follows： the left posterior tibial artery is blocked in Conventional Treatment Group,the left posterior tibial artery is unobstructed in Stem Cell Transplantation G
