摘　　要：目的探讨四川省肿瘤医院院科二级管理实施3年对降低手术超申请量用血的影响。方法通过医院信息中心和中心血库每月统计的数据库中提取数据,回顾分析统计2014—2016年手术用血人次,超申请量用血人次、全院手术总台数。结果近3年手术用血人次占比分别为7.3%（765/10 428）、6.4%（751/11 665）、5.7%（761/13 411）,超申请量用血率为：0.98%（102/10 428）、0.83%（97/11 665）、0.54%（73/13 411）,P〈0.05;超申请量用血人次占比为13.33%（102/765）、12.92%（97/751）、9.59%（73/761）,P〈0.05。结论通过院科二级管理,超申请量用血人次及超申请量用血率逐年降低,总体控制效果较好,继续贯彻落实相关管理制度。Objective To evaluate the impact of a hospital-department blood management system in a three-year test period on reducing over-ordered blood in transfusion operations.Methods Data was collected from both the hospital information center and blood center monthly records ranging from January 1,2014 to December 31,2016.The total number of operations,surgical transfusion,over-ordered blood volume were analyzed. Results The number and the rates of the patients requiring surgical blood transfusion from 2014 to 2016 were 7. 3%（ 765/10 428）,6. 4%（ 751/11 665） and 5. 7%（ 761/13 411）,respectively. The over-ordered blood volume over-ordering rates decreased over the years from 0. 98%（ 102/10 428） to 0. 83%（ 97/11 665） and to 0. 54%（ 73/13 411）（ P 〈0. 05）,The rates of blood over-ordering cases were 13. 33%（ 102/765）,12. 92%（ 97/751）,9. 59%（ 73/761）（ P 〈0. 05）.Conclusion This novel management system helped improving blood management by reducing over-ordered blood. We ought to keep modifying the system for enhanced effects.
