摘　　要：目的运用二维斑点追踪技术测量AR患者左心室整体应变,分析AR患者左心室收缩功能的改变。方法选取中度及以上AR患者32例,根据LVEF分为LVEF正常组(A组)和LVEF减低组(B组)。另选取22例健康志愿者作为对照组。测量左心内径、容积及质量指数等,应用二维斑点追踪技术测量左心室整体纵向应变(GLS)、总体径向应变(GRS)和总体圆周应变(GCS)。应变参数用左心室容量(LVEDV)标化以校正形变的容量依赖性。结果 A组与对照组相比,仅GLS明显减低,GRS与GCS在两组间差异不显著,校正后各应变参数较对照组明显减低;B组与对照组及A组相比,校正前、后各应变参数均明显减低。校正后,应变参数在三组的分布区域明显不同,对照组最高,A组次之,B组最低。AR患者校正后的应变参数与左心室内径、容量以及质量指数显著相关。结论慢性AR患者LVEF正常时,校正后的各项应变参数均已明显减低,且减低早于LVEF。Objective To evaluate the left ventricular systolic function by two-dimensional speckle-tracking echocardiography in patients with chronic aortic regurgitation.Methods Thirty-two patients with chronic AR were studied,along with22 healthy controls.Dividing the AR patients into A group（normal LVEF）and B group（decreased LVEF）.Global longitudinal strain（GLS）,global circumferential strain（GCS）and global radial strain（GRS）were calculated using two-dimensional speckle-tracking echocardiography technique,in addition to left ventricular dimension,volume and mass index.Strain values were normalized to end-systolic volume（LVEDV）to correct for the volume dependency of deformation.Results Compared with healthy controls,only GLS decreased significantly in both A group and B group,GRS and GCS only differed in B group.GLS,GRS and GCS had significant difference between A group and B group.Differences between patients and controls were evident in GLS,GRS and GCS when normalized to LVEDV.All normalized strain variables demonstrated an apparent different in distribution.The normalized strain variables had a good correlation with the left ventricular dimension,volume and mass index.Conclusions The normalized strain variables of asymptomatic chronic AR patients with preserved LVEF decreased,which preceded the decrease of LVEF.
