摘  要:目的探讨声辐射力脉冲成像技术(AFRI)在儿童过敏性紫癜肾炎(HSPN)中的应用价值。方法选取110例病理证实为HSPN的患儿作为研究对象,按不同病理类型分为1、2、3、4、5组,同时选取30例健康体检者作为对照组,应用超声触诊组织定量技术(VTQ)检测各组肾皮质的横向剪切波速度(SWV),进行统计分析。结果 1组的诊断界点为1.70 m/s(灵敏度90%、特异度87%),2组的诊断界点为1.97 m/s(灵敏度100%、特异度74%),3组的诊断界点为2.33m/s(灵敏度100%、特异度84%),4组的诊断界点为2.62m/s(灵敏度90%、特异度70%),5组的诊断界点为2.85m/s(灵敏度80%、特异度80%)。各组HSPN与对照组相比,差异均有统计学意义(P<0.05);各组HSPN间比较,差异具有统计学意义(P<0.05)。各组HSPN肾皮质损害程度及肾脏组织纤维化程度与SWV值间呈正相关(P<0.05)。结论声辐射力脉冲成像可以无创地反映肾脏组织弹性及硬度,对诊断儿童过敏性紫癜肾炎具有一定的临床价值。Objective To investigate the value of acoustic radiation force imaging in children with Henoch-Schonlein nephritis.Methods A total of 110 cases of Henoch-Schonlein nephritis were divided into 5 groups:1 group(typeⅠ),2 group(typeⅡ),3 group(Ⅲtype),4 group(Ⅳtype),5 group(typeⅤ).And other 30 health subjects were taken as control group.The shears wave velocity(SWV)was measured.The SWV values were compared among different groups.The correlation between clinical classification of the 110 cases and SWV was analyzed.Results The diagnostic points of 1,2,3,4,5 were 1.70 m/s(sensitivity was 90%,specificity was 87%),1.97 m/s(sensitivity was100%,specificity was 74%),2.33 m/s(sensitivity was 100%,specificity was 84%),2.62 m/s(sensitivity was90%,specificity was 70%),and 2.85 m/s(sensitivity was 80%,specificity was 80%).There were statistically significant differences between other groups and control group.And there were statistically significant differences in all groups of Henoch-Schonlein nephritis.There was a significant positive correlation between the degree of impairment of renal function and SWV.Conclusions Virtual touch tissues quantification technique can reflect the renal tissue elasticity,It has clinical value in the diagnosis of Henoch-Schonlein nephritis in children.
