摘　　要：目的探讨卡波西样血管内皮瘤(KHE)的超声诊断特征。方法对36例KHE患儿纳入研究,其中10例合并卡梅现象(KMP)。病变累及颌面颈部、躯干及四肢。总结超声特点,包括病灶深度、回声、形态、边界、血管密度、三维彩色形态,并对比分析KHE组与KHE合并KMP组的超声特点。结果 KHE病灶厚度约(1.60±0.83)cm,超声下显示混合回声,高低回声混杂,47.2%低回声为主,52.8%高回声为主,44.4%病灶内见管腔样无回声区,所有病灶内部回声不均匀,91.7%边界不清,41.7%累及肌层。彩色多普勒显示83.3%丰富血流信号,16.7%一般血流信号;三维彩色多普勒显示,KHE呈现由深层至浅层的树枝状血流。KHE合并KMP组对比KHE组,病灶更深,血供更丰富。结论 KHE声像图表现具有一定的特征性,超声检查有助于鉴别诊断。Objective This study evaluated ultrasonography（US）features of kaposiform hemangioendothelioma（KHE）.Methods Between pathologically proven KHE（n=36[10 KHE＋Kasabach-Merritt phenomenon（KMP）]）were reviewed.US findings were retrospectively evaluated.The locations of the lesions were involved in the neck,trunk and extremities.On US images,the size,depth,echotexture,echogenicity,margin,vessel density,blood flow velocity and 3-dimension color Doppler pattern were recorded.The ultrasonic characteristics of KHE and KHE combined KMP group were comparing analyzed.Results The depth of KHE was（1.60±0.83）cm,the lesions were mixed echoic with hyper-and hypo-echo,47.2% exhibited hypoechoic mainly,52.8% hyperechoic mainly,44.4% exhibited visible vessels,all lesions were heterogeneous with 91.7%ill-defined,and 41.7% KHEs exhibited an infiltrative pattern extending into adjacent muscles.83.3% KHEs showed well vascularization,16.7% moderate.The 3-dimension color Doppler revealed branch-shape blood flow for KHE.The KHE＋KMP were significantly deeper and better vascular than that in KHE.Conclusions The ultrasonographic changes of KHE had some certain characteristics,which would be helpful in differential diagnosis of the disease.
