摘 要:目的运用多普勒超声测定高原地区正常妊娠胎儿的脐动脉血流指标,探寻脐动脉血流变化与孕周的相关性。方法选取2016年6—12月在青海大学附属医院(海拔2 300 m)进行产前检查的106例正常妊娠孕妇为研究对象,根据孕周将其分为4组,A组:≤27^(+6)周,42例;B组:28~31^(+6)周,18例;C组:32~35^(+6)周,21例;D组:36~40周,25例。运用多普勒超声对胎儿脐动脉血流指标阻力指数、搏动指数、收缩期峰值血流速度与舒张期峰值血流速度(S/D)比值进行测定。采用Pearson相关系数描述正常妊娠胎儿脐动脉血流指标随孕周变化的相关程度。结果 Pearson检验分析结果显示,高原地区正常妊娠胎儿脐动脉血流阻力指数与孕周呈负相关(相关系数r=-0.405,P<0.001);搏动指数与孕周呈负相关(相关系数r=-0.503,P<0.001);S/D比值与孕周呈负相关(相关系数r=-0.597,P<0.001)。结论高原地区正常妊娠胎儿脐动脉血流阻力指数、搏动指数及S/D比值均与孕周呈负相关,随孕周增加而降低。Objective To investigate the relation between umbilical artery blood flow monitored by Doppler ultrasound and gestational age of healthy fetuses in plateau region. Methods A total of 106 healthy pregnant women were enrolled in Affiliated Hospital of Qinghai University(2 300 m above sea level) from June to December 2016; they were divided into group A(gestational age≤27+6 weeks, n=42) group B(28-31+6 weeks, n=18), group C(32-35+6 weeks, n=21) and group D(36-40 weeks, n=25). Fetal umbilical artery blood flow resistant index, pulsatility index and the ratio of peak systolic velocity(S) to peak diastolic velocity(D) were monitored by Doppler ultrasound. Relation between umbilical artery blood flow parameters and gestational age was analyzed by Pearson test. Results Umbilical artery blood flow resistance index was negatively correlated with gestational age(r=-0.405, P<0.001); pulsatility index was negatively correlated with gestational age(r=-0.503, P<0.001); S/D ratio was negatively correlated with gestational age(r=-0.597, P<0.001). Conclusion In plateau region, resistance index, pulsatility index and S/D ratio of fetal umbilical artery blood flow are negatively correlated with gestational age.
