摘 要:目的:观察颈椎病模型兔造模前后及牵伸松调法干预前后IL-6、TNF-a及Caspase-3含量的变化,探讨手法防治颈椎病的作用机制。方法:40只新西兰健康成年白兔按体重大小排序,用配伍组设计随机分为正常对照组、模型组、手法预防组、手法治疗组,每组10只,采用特制颈椎固定器加风寒湿刺激造模,双抗体夹心酶联免疫吸附法(ELISA)检测造模前、造模后/治疗前和治疗后兔C3~7椎间盘组织和颈伸肌群IL-6、TNF-a及Caspase-3含量的变化。结果:造模后,造模组与手法治疗组家兔椎间盘组织和颈伸肌群IL-6、TNF-a及Caspase-3含量明显升高(P<0.05);治疗后,手法治疗组家兔椎间盘组织和颈伸肌群IL-6、TNF-a及Caspase-3含量显著降低(P<0.05)。手法预防组因造模过程中同时实施了手法预防,造模前/治疗前、后各项指标改变无显著差异(P>0.05)。结论:牵伸松调法能预防和治疗颈椎病,其可能的作用机制与降低椎间盘组织及颈伸肌群IL-6、TNF-a水平,抑制Caspase-3的表达有关。Objective: To observe the effect before and after modeling rabbits as well as before and after stretching-relaxation -adjustment manipulation on IL-6, TNF-α and Caspase-31evels in cervical spondylopathy and to explore its mechanism. Meth- ods: Forty New Zealand grown rabbits were divided by randomized block design into normal group, model group, manipulative prevention group and treatment group averagely according to its weight, special cervical fixator and wind-cold-dampness stimula- tion for modeling two months, the levels of IL-6, TNF-a and Caspase-3in the posterior neck muscles and discs were tested by ELISA method before and after modeling as well as after treatment. Results: The levels of IL-6, TNF-α and Caspase-3in the posterior neck muscles and discs of the model group and the treatment one were increased obviously after modeling well the same indexes were decreased obviously after the treatment (P〈0. 05), and there was no significant difference about the indexes in the manipulative prevention group because of manipulation was being used during the modeling course before and after the modeling as well as the treatment. Conclusion: The stretching-relaxation-adjustment manipulation can prevent and treat the cervical spondy- lopathy, the mechanism is probably related with decreasing the levels of IL-6, TNF-α as well as inhibiting of Caspase-3.
