疏血通注射液联合加味桃红四物汤预防老年骨质疏松压缩性骨折PKP术后静脉血栓的疗效及对D-二聚体的影响

摘　　要：目的观察疏血通注射液联合加味桃红四物汤预防老年骨质疏松压缩性骨折(OVCF)经皮椎体后凸成形术(PKP)术后静脉血栓的疗效及对D-二聚体的影响并探讨其作用机制。方法 150例患者随机分为两组,对照组术后给予常规西医抗凝治疗,观察组在对照组基础上术后当天给予疏血通注射液联合加味桃红四物汤治疗,疗程为14 d。观察两组治疗前、治疗第14天的疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI);观察两组下肢静脉血栓发生情况并检测血清D-二聚体变化情况。结果两组治疗第14天的VAS评分、ODI与治疗前比较均降低,且治疗后观察组VAS评分、ODI评分均低于对照组(均P<0.05)。观察组在治疗第14天静脉血栓发生率4.00%低于对照组的8.00%(均P<0.05)。两组治疗第14天血清D-二聚体水平与治疗前比较均降低,且治疗后观察组血清D-二聚体水平低于对照组(均P<0.05)。结论疏血通注射液联合加味桃红四物汤可显著缓解老年OVCF接受PKP术后患者疼痛症状,改善血液高凝状态,降低术后D-二聚体含量,预防术后静脉血栓形成。Objective： To observe the effect of Shuxuetong Injection and Taohong Siwu Decoction in preventing postoperative venous thrombosis after osteoporotic compression fractures（OVCF） and percutaneous kyphoplasty（PKP） and its effect on D-dimer. Methods： A total of 150 elderly patients with OVCF treated with PKP were randomly divided into the observation group（75 cases） and the control group（75 cases）. The control group received routine Western anticoagulant therapy after operation. On the basis of the control group,Shuxuetong Injection combined with Taohong Siwu Decoction was given to the observation group on the day after the operation for14 days. Visual acuity scores（VAS） and Oswestry Disability Index（ODI） were measured before treatment and on the 14 th day of treatment in both groups. The incidence of venous thrombosis in both groups was observed and the changes of serum D-dimer were observed. Results： The VAS scores and ODI of the 14 th day of treatment in both groups were significantly decreased;the above indexes in the observation group were lower than those in the control group（all P〈0.05）. The incidence of venous thrombosis in the observation group was significantly lower than that in the control group. Serum D-dimer levels decreased significantly on the 14 th day in both groups,and the above indexes in the observation group were lower than those in the control group（all P〈0.05）. Conclusion：Shuxuetong Injection combined with Taohong Siwu Decoction can significantly relieve pain symptoms,improve the state of blood hyper-coagulation,reduce the postoperative D-dimer content and prevent postoperative venous thrombosis in aged OVCF patients receiving PKP,which is worthy of clinical promotion.
