摘  要:目的 对T2DM女性患者残余尿量(RUV)与美国泌尿协会症状指数评分(AUA-SI评分)及其相关因素进行分析,探讨简便有效的方法来初步筛查女性糖尿病神经性膀胱(DNB)。方法 选取我院内分泌科住院T2DM女性患者256例,根据RUV分为RUV 0ml组109例,RUV 0~50ml组84例,RUV>50ml组63例。对各组进行AUA-SI评分及相关指标检测。结果 AUA-SI评分在RUV>50ml组、RUV 0~50ml组与RUV 0ml组依次降低[(21.1±7.9)vs(10.2±4.3)vs(2.4±2.2)分,P<0.01]。Pearson相关分析显示,AUA-SI评分与RUV呈正相关(r=0.578,P<0.05)。AUA-SI评分、RUV分别与病程、FPG、HbA1c、尿微量白蛋白(UmAlb)呈正相关(P<0.05或P<0.01)。结论 AUA-SI评分可作为女性DNB初步筛查的辅助工具。DNB与长期血糖控制不良、糖尿病微血管并发症密切相关。Objective To assess the correlation between residual urine volume(RUV)and AUA-SI score and to find simple and practical tools for initial screening of diabetic neurogenic bladder(DNB)in female type 2 diabetes(T2 DM). Methods 256 female patients with T2 DM were divided into 3 groups according to RUV:109 in RUV 0 ml group,84 in RUV 0~50 ml group and 63 in RUV〉50 ml group.AUA-SI score,glucose and lipid metabolism,urinary albumin were detected and analyzed. Results AUA-SI scores decreased sequentially in RUV〉50 ml group,0~50 ml group and 0 ml group[(21.1±7.9)vs(10.2±4.3)vs(2.4±2.2)score,P〈0.01].Pearson correlation analysis showed a positive correlation between AUA-SI score and RUV(r=0.578,P〈0.05).AUA-SI score and RUV were positively correlated with HbA1 c and urinary microalbumin(UmAlb)(P〈0.05 or P〈0.01). Conclusion AUA-SI score could be considered as a tool for initial screening of DNB in female diabetic patients.The occurrence of DNB is closely related to poor long-term glycemic control and diabetic microvascular complications.
