摘  要:目的 探讨T2DM患者绝经后Dickkopf-1(Dkk1)蛋白表达水平与骨代谢的关系。方法 收集新疆石河子地区绝经后T2DM女性50例(T2DM组)与糖耐量正常(NGT组)女性50名,检测临床代谢指标。ELISA测定血清Dkk1蛋白表达水平。结果 与NGT组比较,T2DM组FPG、2hPG、果糖胺(FMN)、HbA1c、血P水平升高[(5.47±0.73)vs(9.26±2.38)mmol/L,(6.37±0.62)vs(16.66±3.27)mmol/L,(239.74±36.54)vs(337.66±87.31)μmol/L,(6.23±0.92)%vs(8.70±1.76)%,(1.11±0.12)vs(1.24±0.16)mmol/L;P<0.01];骨钙素(OC)、25羟维生素D3[25(OH)D3]、骨密度[BMD(Femur)]水平降低[(30.25±7.41)vs(19.27±5.07)ng/ml,(35.37±6.83)vs(22.00±5.26)nmol/L,0.87(0.80,0.95)vs 0.80(0.61,0.90)g/cm2;P<0.01];Dkk1蛋白表达水平升高[(209.37±32.35)vs(232.97±39.31)pg/ml,P<0.05]。相关分析显示,Dkk1蛋白表达与血P呈正相关(r=0.301,P<0.05),与BMD(Femur neck)呈负相关(r=-0.349,P<0.01)。BMD(Femur)与FPG呈负相关,与OC呈正相关(r=-0.344、0.267,P<0.01)。多元线性回归分析显示,25(OH)D3是BMD(Femur)的影响因素。结论 Dkk1蛋白的高表达可能参与了T2DM患者绝经后BMD降低及血P升高,OC、25(OH)D3等骨代谢指标降低,以此为靶点,有利于防治骨质疏松(OP)。Objective To analyze the relationship between Dickkopf-1(Dkk1)protein expression and bone metabolism in postmenopausal women with type 2 diabetes mellitus(T2 DM). Methods A total of 100 subjects were collected in Shihezi area in Xinjiang province and divided into two groups:patient with T2 DM(T2 DM group,n=50)and healthy subjects(NGT group,n=50).Clinicalin formation was measured and recorded.The expression of serum Dkk1 protein was determined by enzyme linked immunosorbent assay(ELISA). Results The level of FPG,2 hPG,Froctosamine(FMN),HbA1 c and P were higher in T2 DM group than in NGT group[(5.47±0.73)vs(9.26±2.38)mmol/L,(6.37±0.62)vs(16.66±3.27)mmol/L,(239.74±36.54)vs(337.66±87.31)μmol/L,(6.23±0.92)% vs(8.70±1.76)%,(1.11±0.12)vs(1.24±0.16)mmol/L;P〈0.01].The level of Osteocalcin(OC),25(OH)D3,BMD(Femur)were lower T2 DM group than in NGT group[(30.25±7.41)vs(19.27±5.07)ng/ml,(35.37±6.83)vs(22.00±5.26)nmol/L,0.87(0.80,0.95)vs 0.80(0.61,0.90)g/cm^2;P〈0.01].The expression of Dkk1 proteinwas higher in T2 DM group than in NGT group[(209.37±32.35)vs(232.97±39.31)pg/ml,P〈0.05].Correlation analysis showed that the expression of Dkk1 protein was positively correlated with P(r=0.301,P〈0.05)and negatively correlated with BMD(Femur neck)(r=-0.349,P〈0.01).BMD(Femur)was negatively correlated with FPG and positively related with OC(r=-0.344,0.267,P〈0.01).Multiple linear regression showed that 25(OH)D3 was a risk factor for BMD(Femur). Conclusion The high expression of Dkk1 protein may be involved in the decrease of BMD,the increase of P and the decrease of OC and 25(OH)D3 in postmenopausal women with T2 DM.Dkk1 protein may be a target for the prevention and treatment of osteoporosis(OP)in postmenopausal women with T2 DM.
