成人型多囊肾感染囊肿的辨识及治疗

摘　　要：目的探讨成人型多囊肾继发感染个体囊肿的辨识及更合理的治疗方法。方法多囊肾的囊肿用彩超进行反复扫查,找出疑似感染的个体囊肿在彩超引导下抽吸后硬化。结果成人型多囊肾若不合并出血和感染,个体囊肿超声表现为:界限清楚,形态饱满,呈圆形或椭圆形,囊壁光滑,囊液呈无回声。合并感染的囊肿,超声表现为:界限欠清,囊壁毛糙,形态欠饱满,囊液呈细小强回声光团,少数囊液内合并纤维光带,囊肿周围血流丰富。感染的囊肿抽吸后患者体温下降,全身使用抗菌素后,血象回复正常。抽出未感染的囊液透明状,无色无味。合并感染的囊液呈混浊状,颜色呈淡黄色,黄色和脓样变。部分有恶臭。结论合并感染的囊肿个体超声介入是最好的方法之一。没有合并感染的囊肿,可以择期处理。合并感染的囊肿必须及时合理处理。超声检查能准确辨识感染的囊肿个体并且能够精确引导下引流硬化治愈。Objective To identify individual cysts in adult polycystic kidney infection and a more reasonable treatment method. Methods The cysts of polycystic kidney were repeatedly scanned with color Doppler ultrasound to find individual cysts that are suspected to be infected and then sclerotherapy after suction. Results If there is no bleeding and infection in adult polycystic kidney,the ultrasonographic features of individual cyst are： clear boundary,full shape,round or oval shape,smooth cystic wall,and no echo in cystic fluid. Infected cyst,ultrasound showed： boundary less clear,cystic wall rough form,under full,cystic fluid was tiny echogenic cystic fluid with fiber,a light,rich blood flow around the cyst. After suction and sclerosis of infected cysts,the patient＇s body temperature drops,and after the whole body uses antibiotics,the blood picture returns to normal. Extract uninfected cystic fluid transparent,colorless and tasteless.The cystic fluid of the combined infection is cloudy,the color is pale yellow,yellow and purulent,and part of the stench. Conclusion Ultrasound intervention is one of the best methods for an infected cyst. Cysts without infection can be treated with a selective treatment. The cysts associated with infection must be treated in a timely and reasonable manner. Ultrasound can accurately identify the infected cyst individual and accurately guide the drainage sclerosis to cure.
