摘 要:目的探讨层级管理在老年医院养老护理员中的应用效果。方法采用目的抽样法选择湖南省人民医院马王堆院区246名养老护理员为研究对象,于2016年1—6月对养老护理员进行层级管理培训,于培训后采用核心胜任力量表、自制考核试卷、护理质量满意度问卷进行调查分析。结果培训后,养老护理员核心胜任力得分为(425.17±14.50)分,高于培训前(398.79±23.29)分,差异有统计学意义(t=15.464,P〈0.001);理论与技能考核成绩得分分别为(77.96±4.61)、(84.73±6.24)分,均高于培训前(70.06±6.12)、(80.34±6.02)分,差异均有统计学意义0值分别为16.165、7.762;P〈0.001);培训后养老护理员所在科室护士长与住院老年患者对其护理服务质量满意度提高,差异有统计学意义(P〈0.001)。结论对养老护理员进行护理层级管理评估和内容规范化培训,能有效提升养老护理员核心胜任力、理论与技能水平及住养老人和护士长满意度,改善护理质量。Objective To explore the effects of grading management among elderly caregivers in the geriatric hospital. Methods We selected 246 elderly caregivers as subjects in Mawangdui District of HuMan Provincial People's Hospital by purposive sampling. From January 2016 to June 2016, elderly caregivers received grading management training. After training, they were investigated with the core competency scale, self-designed assessment examination paper, nursing quality satisfaction questionnaire. Results The score of core competency of elderly caregivers was (425.17 ± 14.50) after training higher than that (398.79± 23.29) before training with a significant difference (t=15.464, P 〈 0.001). The theoretical achievement and skills performance was (77.96 ± 4.61) and (84.73 ± 6.24) respectively after training significantly higher than those [ (70.06 ±6.12), (80.34 ± 6.02) ] before training (t=16.165, 7.762; P 〈 0.001). After training, the satisfaction of head-nurses and elderly inpatients with nursing service quality of elderly caregivers all increased with significant differences (P 〈 0.001). Conclusions The application of grading management nursing assessment and standardized training of content for elderly caregivers can effectively improve nursing assistants' core competency, theoreticaland skills level, satisfaction of head-nurses and elderly inpatients as well as nursing quality.
