摘　　要：目的优化St Mary＇s气管切开患者集束化护理方案，并评价其临床应用效果。方法采用专家会议法优化St Mary＇s气管切开集束化护理方案，2016年11月-2017年7月选取耳鼻咽喉科、神经外科100例气管切开住院患者为研究对象，采用Epidata软件随机分为两组，每组50例。观察组实行集束化护理方案，对照组实行常规护理。比较两组患者平均带管时间、住院费用及并发症发生率，评价临床效果。结果观察组完成各项护理措施总体依从性为82．0％，最终纳人31例患者。观察组并发症发生率为6．5％，对照组为26．0％，差异有统计学意义（χ2=4．846，P〈0．05）。观察组气管套管留置时间（23．03±6．22）d，总住院费用（19804．00±4401．70）元，均低于对照组，差异有统计学意义（t值分别为-4．250、-8．570；P〈0．01）。结论优化后St Mary＇s集束化护理方案临床实施效果较好，具有临床推广应用价值。Objective To optimize the St Mary＇s cluster nursing care after tracheotomy and evaluate it＇s clinical effectiveness. Methods St Mary＇s cluster nursing care after traeheotomy was optimized by using expert meeting method. From November 2016 to July 2017, a total of 100 hospitalized patients undergoing tracheotomy from Department of Otolaryngology and Department of Neurosurgery were selected as the research object. They were randomly divided into two groups by using Epidata software, 50 patients in each group. Patients in the observation group received cluster nursing care. And patients in the control group received conventional care. The average intubation duration, hospitalization expenses and incidence of complications were compared between the two groups, and the clinical effects were evaluated. Results The overall compliance of the observation group was 82.0%, which included 31 patients. The complication rate of the patients in the observation group was 6.5%, and the control group was 26.0%, the difference was statistically significant （ χ2=4-846, P 〈 0.05）. The retention time of the tracheal tube in the observation group was （23.03 ±6.22） d and the total hospitalization cost （19 804.00 ± 4 401.70）yuan, which were all lower than those of the control group （t=-4.250, -8.570; P〈0.01）. Condusions After optimization, the St Mary＇s cluster nursing care after tracheotomy has better clinical effect, which makes it more valuable in clinical application.
