综合康复措施在高龄女性压力性尿失禁患者护理中的应用效果

摘　　要：目的探讨综合康复措施对高龄女性轻、中度压力性尿失禁（SUI）患者尿动力学、临床症状和生活质量的影响。方法2016年5月-2017年5月，按照随机数字表法将60例高龄女性SUI患者分为观察组和对照组，每组30例，对照组给予常规护理，观察组实施综合康复措施，包括心理疏导、膀胱功能训练、Kegel锻练和电刺激治疗等。评估两组患者最大尿道压（MUP）和最大尿道关闭压（MUCP），采用尿失禁问卷简表（ICIQ—SF）和生活质量问卷（I-QOL）比较两组康复效果。结果观察组患者在综合康复3个月后，MUP（46．11±3．71）cmH2O，MUCP（37．65±4．62）cmH2O，均高于对照组，差异均有统计学意义（t值分别为3．397、3．128；P〈0．01）。干预3个月后，两组患者ICIQ-SF和I-QOL评分均有改善；观察组ICIQ—SF（17．52±1．61）分，I-QOL（81．11±8．61）分，均优于对照组，差异有统计学意义0值分别为-2．472、2．614；P〈0．05）。结论综合康复措施是高龄女性轻、中度SUI患者的有效治疗手段，值得临床推广应用。Objective To investigate effects of comprehensive rehabilitation measures on urodynamic, clinical symptoms and quality of life in mild to moderate stress urinary incontinence （SUI） of aged female. Methods A total of 60 SUI aged women were randomized divided into either control group （30 patients, on conventional nursing） or intervention group （30 patients, on conventional nursing, combined with psychological counseling, bladder exercise, Kegel exercise, and electrical stimulation）. The maximal urethral pressure （MUP） and maximal urethral closing pressure （MUCP） were examined and used for evaluating the interventioneffects, in combination with the questionnaire of international consultation on Incontinence Questionnaire Short Form （ICIQ-SF） and Incontinence Quality of Life Scale （I-QOL）. Results At 3 months after the comprehensive rehabilitation, the MUP [ （46.11 ± 3.71） cmH2O ] and MUCP [ （37.65 ± 4.62） cmH20 1 in the intervention group were higher than control group （t=3.397, 3.128, P 〈 0.01）. ICIQ-SF and I-QOL scores in both groups were improved. ICIQ-SF （17.52± 1.61） and I-QOL （81.11 ± 8.61） scores in the intervention group were significantly higher than control group （t=-2.472, 2.614, P 〈 0.05）. Conclusions Comprehensive rehabilitation is an effective intervention to treat mild to moderate SUI aged female patients.
