摘　　要：目的探讨医院-社区-家庭联动模式在改善冠心病患者自我管理行为中的应用。方法选择2016年7—12月哈尔滨市某三级甲等医院住院的冠心病患者130例，按照患者住院先后顺序进行分组，观察组65例，对照组65例。观察组采用医院-社区-家庭联动模式进行干预，对照组采用常规医院健康教育模式，干预前后采用冠心病自我管理量表比较两组患者的自我管理行为。结果出院6个月后观察组患者日常生活管理（16．84±4．46）分、不良嗜好管理（15．94±4．15）分、疾病知识管理（16．97±4．23）分、症状管理（13．55±4．32）分、急救管理（12．64±4．19）分及治疗依从性管理（10．86±3．54）分，均高于对照组，差异有统计学意义“值分别为3．148、2．716、2．476、2．384、2．876、4．427；P〈0．05）。结论医院-社区-家庭联动模式可以改善冠心病患者的自我管理行为，值得推广应用。Objective To explore the effect of hospital-community-family linkage model on improving the self-management behavior of patients with coronary heart disease. Methods A total of 130 patients with coronary heart disease who were hospitalized in a Class Ⅲ Grade A hospital in Harbin were investigated from July to December 2016. Patients were divided into the observation group （65 cases） and control group （65 cases） according to the hospitalization order. The hospital-community-family linkage model was used in the observation group, while conventional hospital health education model was applied in the control group. The self- management behavior of patients in two groups was investigated using Coronary Self-Management Scale （CSMS）. Results At 6 months after discharge, the scores of daily life management, had habits management, disease knowledge management, symptom management, emergency management and treatment compliance management in the observation group were （16.84 ±4.46）, （15.94 ±4.15）, （16.97 ± 4.23）, （13.55± 4.32）, （12.64 ± 4.19）, and （10.86 ±3.54）, which were significantly higher than the control group （t=3.148, 2.716, 2.476, 2.384, 2.876,4.427; P 〈 0.05）. Conclusions Hospital-community-family linkage model can improve the self-management behavior of patients with coronary heart disease, which is worthy of promotion and application.
