白血病患儿细致性护理对心理状态及并发症的影响

摘　　要：目的：研究细致性护理对白血病患儿心理状态与并发症的影响。方法：依据随机数字表法将我院74例白血病患儿分为两组,各37例,对照组给予常规护理,研究组基于常规护理实施细致性护理干预,比较两组护理前后抑郁（SDS）、焦虑（SAS）、症状自评量表（SCL-90）、生活质量（PedsQLTM）评分、依从性及并发症情况。结果：研究组SDS评分（t=-6.696,P〈0.001）、SAS评分（t=-6.125,P〈0.001）明显低于对照组;研究组SCL-90精神病性（t=-9.867,P〈0.001）、躯体化（t=-9.578,P〈0.001）、偏执（t=-6.622,P〈0.001）、强迫症（t=-5.365,P〈0.001）、敌对性（t=-6.698,P〈0.001）、恐怖（t=-9.405,P〈0.001）、人际关系（t=-7.887,P〈0.001）、抑郁（t=-8.773,P〈0.001）、焦虑（t=-7.112,P〈0.001）因子评分均明显低于对照组;研究组PedsQLTM3.0总分（t=10.716,P〈0.001）、PedsQLTM4.0总分（t=10.532,P〈0.001）、依从率（χ~2=4.723,P=0.030）均明显高于对照组;研究组并发症总发生率（χ~2=5.736,P=0.017）显著低于对照组。结论：对白血病患儿给予细致性护理,可有效改善负面心理及情绪,优化心理状态,提高生活质量与化疗配合度,降低并发症风险,应用价值高。Objective：To investigate the effect of careful nursing on the mental status and complications in children with leukemia.Methods：A total of 74 children with leukemia were divided into two groups by the random number table method,37 cases in each group.The control group were given routine nursing while the study group were given careful nursing intervention based on routine nursing.The scores of depression（SDS）,anxiety（SAS）,symptom checklist 90（SCL-90）and quality of life（PedsQLTM）,compliance and complications were compared between the two groups before and after nursing.Results：The SDS score（t=-6.696,P〈0.001）and SAS score（t=-6.125,P〈0.001）in the study group were significantly lower than those in the control group.Scores of psychosis,somatization,paranoia,obsessive-compulsive disorder,hostility,terror,interpersonal relationship,depression and anxiety in SCL-90 scale of the study group were significantly lower than those in the control group（t=-9.867,-9.578,-6.622,-5.365,-6.698,-9.405,-7.887,-8.773,-7.112;P 0.001）.The total score of PedsQLTM3.0（t=10.716,P 0.001）,the total score of PedsQLTM4.0（t=10.532,P〈0.001）and compliance（χ~2=4.723,P〈0.05）of the study group were significantly higher than those of the control group.The total incidence of complications in the study group（χ~2=5.736,P〈0.05）was significantly lower than that in the control group.Conclusion：Careful nursing to children with leukemia can effectively relieve the negative emotions and mood,optimize the mental status,improve the quality of life and compliance to chemotherapy,and reduce the risk of complications.
