术前头部CT血管造影在脑膜瘤治疗中的应用

摘　　要：目的观察术前头部CT血管造影(CTA)在脑膜瘤治疗中的应用效果。方法回顾性分析50例脑膜瘤患者的临床资料,比较术前CTA检查与术中所见,包括肿瘤大小、基底、供血来源及肿瘤与血管、颅骨关系,并评价模拟骨窗有效性。结果术前CTA检测提示嗅沟脑膜瘤6例,鞍结节脑膜瘤7例,蝶骨嵴脑膜瘤15例,岩斜区脑膜瘤6例,镰旁脑膜瘤6例,窦镰旁脑膜瘤10例。术前CTA提示肿瘤与血管关系及肿瘤血供来源与术中所见比较,差异均无统计学意义(均P>0.05)。术前模拟骨窗大小、骨窗底部离颅底的距离、骨质阻挡等与术中所见亦一致。结论术前CTA能有效分辨脑膜瘤与附近血管关系,清晰显示脑膜瘤血供来源,有效模拟骨窗,为脑膜瘤手术提供直观的信息。Objective To observe the efficacy of preoperative computed tomography angiography（ CTA） applied to meningiomas treatment. Methods The clinical data of 50 patients with meningioma were retrospectively analyzed. The preoperative CTA results were compared with the intraoperative findings in the items of tumor size,the basal,source of blood supply,and the relationship of tumor with vessel and skull. And the effectiveness of simulation bone window was assessed. Results The results of preoperative CTA revealed that there were 6 cases of olfactory groove meningiomas,7 cases of saddle-nodule meningiomas,15 cases of sphenoid ridge meningiomas,6 cases of petroclival meningiomas,6 cases of falcine meningiomas and 10 cases of sinus falcine meningiomas. There was no significant difference between preoperative CTA results and intraoperative findings in the items of the relationship between tumor and vessel or the source of blood supply of tumor（ all P〈0. 05）. The preoperative simulation bone window size,the distance from the base of bone window to the base of skull,and the bone overlap were consistent with the intraoperative findings. Conclusion Preoperative CTA can effectively clarify the relationship between meningioma and surrounding vessels,clearly indicate the source of blood supply of meningioma,and effectively simulate bone window,providing intuitive information for meningioma surgery.
