体位改变对急性前/间壁心肌梗死患者心电图的影响

摘　　要：目的探讨体位改变对急性前间壁和(或)前壁心肌梗死患者心电图的影响。方法选择43例急性前间壁和(或)前壁心肌梗死患者,均进行仰卧位、左侧卧位、右侧卧位、坐位及站位的心电图检查。比较仰卧位与其他体位的V1~V4导联Q波振幅和时限、ST段偏移幅值、T波振幅差异。结果与仰卧位比较,左侧卧位的V2、V3导联Q波振幅减小,坐位的V2导联Q波振幅减小,站位的V2导联Q波时限减小(均P<0.05);左侧卧位、右侧卧位的V2导联ST段偏移值降低,坐位的V2、V3导联ST偏移值降低(P<0.05);右侧卧位的V2导联T波振幅降低,坐位和站位的V3、V4导联T波振幅增高(均P<0.05)。结论体位改变对急性前间壁和(或)前壁心肌梗死患者的心电图有一定影响。Objective To investigate the effect of position change on ECG in the patients with acute anterior/antero-septalmyocardial infarction. Methods Forty-three patients with acute anterior and（ or） antero-septal myocardial infarction were selected,and underwent ECG in different positions including supine,left lateral,right lateral,sitting and standing positions. Q-wave amplitude and duration,ST segment deviation amplitude and T-wave amplitude of leads V1 to V4 were compared between the supine position and the other positions. Results Compared with the supine position,Q-wave amplitudes of leads V2 and V3 decreased in the left lateral position,Q-wave amplitude of lead V2 decreased in the sitting position,Q-wave duration of lead V2 decreased in the standing position（ all P〈0. 05）; ST segment deviation amplitude of lead V2 decreased in the left and right lateral positions,ST segment deviation amplitudes of leads V2 and V3 decreased in the sitting position（ P〈0. 05）; T-wave segment amplitude of lead V2 decreased in the right lateral position,T-wave segment amplitudes of leads V3 and V4 increased in the sitting and standing positions（ all P〈0. 05）. Conclusion Position change obtains a certain effects on ECG in the patients with acute anterior and（ or） antero-septal myocardial infarction.
