摘　　要：目的:了解武汉科技大学附属天佑医院(以下简称"我院")抗凝血药的应用现状,促进抗凝血药的合理应用。方法:对2015—2017年我院抗凝血药的用药频度(defined daily dose system,DDDs)、限定日费用(defined daily cost,DDC)、药品销售金额排序(B)/DDDs排序(A)及各病种、各临床科室的使用情况进行统计分析。结果:2015—2017年,我院药品总销售金额分别为9 010.44万元、9 832.75万元和10 957.82万元,抗凝血药销售金额分别为399.10万元、566.75万元和748.04万元,抗凝血药销售金额占药品总销售金额的比例分别为4.43%、5.76%和6.83%,均呈逐年增长趋势;阿司匹林的DDDs排序稳居第1位,其B/A为2,DDC最低(<0.6元);新型抗凝血药达比加群酯、利伐沙班和比伐卢定等的价格偏高,DDDs排序较为靠后;各类疾病中,用于脑卒中的抗凝血药销售金额最高,且占比较高(约50%);各科室中,神经内科、心血管内科抗凝血药的销售金额最高,两者合计占比达60%以上。结论:我院抗凝血药的使用基本合理,能遵循相关指南和疾病治疗的要求,但应加强合理用药监测,确保用药安全、有效、经济。OBJECTIVE： To investigate the application of anticoagulants in Tianyou Hospital Affiliated to Wuhan University of Science and Technology（ hereinafter referred to as ＂our hospital ＂）,so as to promote the rational application of anticoagulant. METHODS： The defined daily dose system（ DDDs）,defined daily cost（ DDC）,consumption sum（ B）/sequence of DDDs（ A）,application of anticoagulant on different diseases and different clinical departments in our hospital during 2015-2017 were collected and analyzed. RESULTS： During 2015-2017,the total consumption sums in our hospital were respectively 90. 104 4 million yuan,98. 327 5 million yuan and 109. 578 2 million yuan,of which the consumption sum of anticoagulant were respectively 3. 991 0 million yuan,5. 667 5 million yuan and 7. 480 4 million yuan,with the proportions were respectively 4. 43%,5. 76% and 6. 83%,which was in an increasing trend; the DDDs of aspirin was in the first place,with B/A of 2 and the lowest DDC（ less than 0. 6 yuan）;new anticoagulants of dabigatran etexilate,rivaroxaban and bivalirudin were in higher price and low ranking in DDDs;of all kinds of diseases,cerebral apoplexy was the disease that had been applied the highest consumption sum of anticoagulant, with approximately 50%; of all clinical departments, neurology and cardiovascular medicine departments had been applied the highest consumption sum of anticoagulant, with a summation of over 60%.CONCLUSIONS： The application of anticoagulant in our hospital is basically rational,which has met the requirements of related guides and disease treatment,while the monitoring on rational drug application still needs to be reinforced to ensure safe,effective and economic medication.
