支气管哮喘缓解期患者中医辨证分型与免疫指标的关系研究

摘　　要：目的:探讨不同中医证型的支气管哮喘缓解期患者免疫指标的差异。方法:140例支气管哮喘缓解期患者辨证分型为:肺气虚组71例,肺脾气虚组33例,肺肾气虚组36例,比较3组患者各类免疫指标的水平。结果:肺气虚组患者CD_3^+高于肺脾气虚组和肺肾气虚组(P<0.05);3组患者CD_4^+、CD_8^+水平比较,差异无统计学意义(P>0.05);肺气虚组患者IgE、IgM、IgA、白细胞介素-4(interleukin-4,IL-4)显著低于肺脾气虚组、肺肾气虚组,差异均有统计学意义(P<0.05);肺脾气虚组患者IL-4显著低于肺肾气虚组(P<0.05);肺气虚组患者干扰素-γ(interferon-γ,INF-γ)水平高于肺脾气虚组和肺肾气虚组(P<0.05);肺脾气虚组的INF-γ水平高于肺肾气虚组(P<0.05);3组患者IgG水平比较,差异无统计学意义(P>0.05)。结论:不同中医证型的支气管哮喘缓解期患者的免疫水平有差异:肺气虚组患者肺功能优于肺脾气虚组、肺肾气虚组患者;肺气虚组患者CD_3^+水平高于肺脾气虚组、肺肾气虚组患者;肺气虚组患者IgE、IgM、IgA、IL-4水平均低于肺脾气虚组、肺肾气虚组患者;肺气虚组患者INF-γ水平高于肺脾气虚组、肺肾气虚组患者;肺肾气虚组患者INF-γ水平低于肺脾气虚组患者,而IL-4水平高于肺脾气虚组患者。Objective： To explore the difference of immune index of bronchial asthma in remission stage and different TCM syndrome type. Methods： 140 patients with bronchial asthma who achieved remission criteria after treatment were selected for retrospective study. The patients were divided into lung-Qi deficiency group（ 71 case）,asthenia of pulmonosplenic Qi group（ 33 case） and insufficiency of Qi of the lung and kidney group（ 36 case）,and various types of immune index of the three groups were tested and compared. Results： The CD3＋ of lung-Qi deficiency group were higher than asthenia of pulmonosplenic Qi group and insufficiency of Qi of the lung and kidney group（ P〈 0. 05）; the CD4＋ and CD8＋ of the three groups had no difference（ P〈 0. 05）. The IgE,IgM,IgA and interleukin-4（ IL-4） index of lung-Qi deficiency group were lower than asthenia of pulmonosplenic Qi group and insufficiency of Qi of the lung and kidney group（ P〈 0. 05）. The IL-4 of asthenia of pulmonosplenic Qi group were lower than insufficiency of Qi of the lung and kidney group（ P〈 0. 05）; the INF-γ of lung-Qi deficiency group were higher than asthenia of pulmonosplenic Qi group and insufficiency of Qi of the lung and kidney group（ P〈 0. 05）,the INF-γ of asthenia of pulmonosplenic Qi group were higher than insufficiency of Qi of the lung and kidney group（ P〈 0. 05）. The IgG of the three groups had no difference（ P〈 0. 05）. Conclusion： The immune level of bronchial asthma patients with different TCM syndromes was different.
