八味妇炎汤治疗不孕症临床研究

摘　　要：目的:观察八味妇炎汤治疗慢性盆腔炎性疾病引起的输卵管炎症性不孕症的临床疗效,以及对炎症因子及血液流变学的影响进行分析。方法:将118例慢性盆腔炎性疾病引起的输卵管炎症性不孕症患者按随机数字表法分为对照组和研究组,每组59例。对照组采用常规西医治疗,研究组在对照组治疗基础上加用八味妇炎汤治疗,两组患者均持续治疗3个月经周期。观察两组患者中医证候积分和临床疗效,检测两组患者血清炎症因子、血液流变学指标和卵巢动脉血流动力学水平变化情况。结果:治疗后,两组患者中医证候积分均较治疗前显著降低,且研究组显著低于对照组,差异均有统计学意义(P<0.05);研究组临床治疗有效率显著高于对照组,差异有统计学意义(P<0.05);两组患者血清白细胞介素-2(interleukin-2,IL-2)、白细胞介素-6(interleukin-6,IL-6)和肿瘤坏死因子(tumor necrosis factor-α,TNF-α)均较治疗前显著降低,且研究组均显著低于对照组,差异均有统计学意义(P<0.05);两组患者纤维蛋白原、红细胞压积和全血低切黏度均较治疗前显著降低,且研究组均显著低于对照组,差异均有统计学意义(P<0.05);两组患者阻力指数和搏动指数均较治疗前显著降低,且研究组均显著低于对照组,差异均有统计学意义(P<0.05)。结论:八味妇炎汤治疗慢性盆腔炎性疾病引起的输卵管炎症性不孕症疗效显著,同时能降低炎症因子水平、改善血液流变学。Objective： To observe clinical effect of Fuyan decoction with eight tastes in treatment of tubal inflammatory infertility caused by chronic pelvic inflammatory disease,and to analyze the influence on the inflammatory cytokines and hemorheology.Methods： 118 cases of patients with tubal inflammatory infertility caused by chronic pelvic inflammatory disease were divided into two groups randomly. The control group with 59 cases were given routine western medicine treatment,and the study group were given Fuyan decoction with eight tastes on the basis of the control group,and all the two groups treated for 3 menstrual cycles. TCM syndrome scores were figured before and after treatment,clinical effect was recorded,and serum inflammatory factors,hemorheology and ovarian arterial hemodynamics were detected before and after treatment in the two groups. Results： After treatment,TCM syndrome scores were significantly lower than those before treatment,which of the study group were significantly lower than those of the control group（ P 〈 0. 05）. The effective rate of the study group was significantly higher than that of the control group（ P〈 0. 05）. Serum interleukin（ IL）-2,IL-6 and tumor necrosis factor（ TNF）-α were significantly lower than those before treatment,which of the study group were significantly lower than those of the control group（ P 〈 0. 05）. Fibrinogen,hematocrit and whole blood low shear viscosity were significantly lower than those before treatment in the two groups,which of the study group were significantly lower than those of the control group（ P 〈 0. 05）. Resistance index and pulsatility index were significantly lower than those before treatment（ P 〈 0. 05）,which of the study group were significantly lower than those of the control group（ P 〈 0. 05）.Conclusion： The effect is obvious of Fuyan decoction with eight tastes in treatment of tubal inflammatory infertility caused by chronic pelvic inflammatory disease,which can reduce the levels of inflammatory
