温胆汤加减联合多烯磷脂酰胆碱胶囊治疗非酒精性脂肪性肝病临床观察  

Clinical Observation of Wendan Decoction Combined with Polyene Phosphatidylcholine Capsule in the Treatment of Nonalcoholic Fatty Liver Disease

摘　　要：目的:观察温胆汤加减联合多烯磷脂酰胆碱胶囊对非酒精性脂肪性肝病(nonalcoholic fatty liver disease,NAFLD)患者临床疗效及血糖、血脂、氧化应激水平的影响。方法:选取2015年6月至2016年6月期间收治的114例NAFLD患者,随机分为对照组、观察组,每组57例,对照组采用多烯磷脂酰胆碱胶囊治疗,观察组采用温胆汤加减联合多烯磷脂酰胆碱胶囊治疗,两组患者均治疗12周。观察治疗后两组NAFLD患者空腹血糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting serum lisulin,FINS)、总胆固醇(total cholesterol,TC)、三酰甘油(triglyceride,TG)、低密度脂蛋白-胆固醇(low density lipoprotein-cholesterol,LDL-C)、高密度脂蛋白-胆固醇(high density lipoprotein-cholesterol,HDL-C)水平,计算胰岛素抵抗指数(HOMA-IR);采用羟胺法检测超氧化物歧化酶(superoxide dismutase,SOD)水平,采用硫代巴比妥酸比色法检测丙二醛(malondialdehyde,MDA)水平,评价临床疗效。结果:治疗后观察组患者有效率为98.25%,显著高于对照组的85.96%,两组比较,差异有统计学意义(P<0.05);两组患者治疗后FPG、FINS、HOMA-IR表达水平均低于治疗前,差异有统计学意义(P<0.05),治疗后观察组患者FPG、FINS、HOMA-IR表达水平分别为(4.86±1.02)mmol·L^(-1)、(6.12±1.42)U·L^(-1)、1.21±0.57,显著低于对照组的(6.02±1.67)mmol·L^(-1)、(8.67±2.54)U·L^(-1)、1.98±0.86,两组比较,差异有统计学意义(P<0.05);治疗后观察组患者TG、TC表达水平分别为(1.56±0.24)mmol·L^(-1)、(4.57±1.52)mmol·L^(-1),显著低于对照组的(2.13±0.84)mmol·L^(-1)、(5.86±2.14)mmol·L^(-1),两组比较,差异有统计学意义(P<0.05),而治疗后两组患者LDL-C、HDL-C表达水平比较,差异无统计学意义(P>0.05);治疗后观察组患者SOD表达水平为(95.26±17.54)U·mL^(-1),显著高于对照组的(82.52±15.47)U·mL^(-1),MDA表达水平为(4.54±1.54)μmol·L^(-1),显著低于对照组的(5.78±2.02)μmol·L^(-1),两组比较,�Objective： To investigate the effect of Wendan decoction combined with Polyene Phosphatidylcholine capsule in the treatment of nonalcoholic fatty liver disease（ NAFLD） and its effects on blood glucose,blood lipid and oxidative stress. Methods：114 patients with NAFLD in our hospital from June 2015 to June 2016 were selected. The patients were randomly divided into two groups with 57 cases in each group. The cases in the control group were given Polyene Phosphatidylcholine capsule,and the cases in the observation group were treated with Wendan decoction combined with Polyene Phosphatidylcholine capsule. The clinical effects of different treatment methods on NAFLD patients and the influence on blood glucose,blood lipid and oxidative stress were compared between two groups. Both groups were treated for 12 weeks. After treatment,the levels of fasting plasma glucose（ FPG）,fasting serum insulin（ FINS）,total cholesterol（ TC）,triglyceride（ TG）,low density lipoprotein cholesterol（ LDL-C） high density lipoprotein cholesterol（ HDL-C） and the insulin resistance index（ HOMA-IR） were observed in two groups of NAFLD patients.The levels of superoxide dismutase（ SOD） and malondialdehyde（ MDAs） were measured by hydroxylamine method and thiobarbituric acid colorimetry respectively. Results： After treatment,the efficiency of the observation group was 98. 25%,which was significantly higher than that in the control group 85. 96%（ P 〈 0. 05）. Before treatment,the expression level of FPG,FINS,HOMA-IR were no significant difference between the two groups（ P 〉0. 05）. After treatment,the FPG,FINS,HOMA-IR expression levels in the observation group were（ 4. 86 ± 1. 02） mmol·L（-1）,（ 6. 12 ± 1. 42） U·L（-1） and（ 1. 21 ± 0. 57） respectively,which were significantly lower than that in the control group [（ 6. 02 ± 1. 67） mmol · L（-1）,（ 8. 67 ± 2. 54） U·L（-1）,（ 1. 98 ± 0. 86）,（ P〈 0. 05） ].
