颈内静脉穿刺中心静脉置管术400例临床体会

摘　　要：目的探讨颈内静脉穿刺中心静脉置管术的临床应用效果。方法回顾性分析沈阳医学院附属中心医院2012年6月-2015年11月400例行中心静脉穿刺置管术患者的临床资料，统计置管成功率、导管留置时间及导管相关并发症发生情况。结果所有患者中心静脉置管均成功；导管留置时间2-495d，平均26d；发生并发症24例，其中误穿动脉致局部血肿3例、导管阻塞6例、感染11例、脱管4例，无气胸及空气栓塞等严重并发症发生。结论颈内静脉穿刺中心静脉置管术的并发症发生率相对较低，留置时间长，是一种较为安全、有效的静脉通路，值得临床推广应用。Objective To explore the clinical application of central venous catheterization via internal jugular vein. Methods The clinical data of 400 patients underging central venous catheterization via internal jugular vein puncture in our hospital were retrospectively analyzed. The success rate of catheterization, catheter retention time and catheter related complications were observed. Results Central venous catheterization was performed successfully for all cases. The catheter was kept for 2 to 495 days, and the average indwelling time is 6 days. All kinds of complications occurred in 24 cases, including 3 cases of local hemorrhage and hematoma because of puncturing the artery inadvertently, 6 case of catheter obstruction, 11 cases of infection, 4 case of catheter inadvertent withdrawal. None of the pneumothorax or other serious complications happened. Conclusion Central venous catheterization via internal jugular vein puncture is a very safe and effective venous channel with fewer complications and longer indwelling duration. It is worth to be widely utilized in clinical practice.
