真武强心汤联合西药治疗老年慢性心力衰竭的疗效研究

摘　　要：目的:探究真武强心汤联合西药治疗老年慢性心力衰竭的临床疗效。方法:选取我院2015年8月-2017年8月收治的慢性心力衰竭患者72例,根据患者自愿原则分为西药治疗(瑞舒伐他汀)的A组(n=24);真武强心汤治疗的B组(n=24)以及真武强心汤与西药联合治疗的C组(n=24),对比分析三组患者治疗前后6MWT(6min步行距离)、左室舒张末期内径(LVEDD)和左室射血分数(LVEF)、TNF-α(肿瘤坏死因子α)、IL-6(白细胞介素6)以及治疗的总有效率。结果:(1)经统计学分析,治疗1个月后,C组治疗的总有效率高达91.67%(22/24),分别高于A、B两组的62.5%(15/24)和79.17%(19/24),总有效率C组>B组>A组,两两比较差异均具有统计学意义(P<0.05)。(2)经统计分析,三组患者治疗前各观察指标差异不具有统计学意义(P>0.05),治疗1个月后,C组6MWT、LVEF显著高于A、B两组,而LVEDD、IL-6、TNF-α显著低于A、B两组,差异具有统计学意义(P<0.05);B组6MWT、LVEF、IL-6、TNF-α显著高于A组,而LVEDD低于A组,差异具有统计学意义(P<0.05)。结论:真武强心汤联合西药治疗老年慢性心力衰竭相对于单纯西药治疗和单纯真武强心汤治疗,效果更佳,能有效改善患者心脏功能,降低炎症反应,提高患者运动能力,值得临床推广。Objective： To explore the clinical effect of the Zhenwu Qiangxin decoction（真武强心汤） with western medicine on chronic heart failure in elderly patients. Methods： 72 patients with chronic heart failure admitted to and treated in our hospital from August 2015 to August 2017 were selected as research subjects and divided into Group A（n=24）, Group B（n=24） and Group C（n=24） according to the voluntary principle of the patients, Group A received western medicine（reshuvastatin）, Group B received the Zhenwu Qiangxin decoction, while Group C received the Zhenwu Qiangxin decoction with western medicine. 6 MWT（6 minutes walking distance）, left ventricular enddiastolic diameter（LVEDD） and left ventricular ejection fraction（LVEF）, TNF alpha（tumor necrosis factor alpha）, IL-6（interleukin 6）, and total effective rate in three groups before and after treatment were compared and analyzed. Results：（1） The statistical analysis showed that one month after treatment, the total effective rate in group C（22/24） 91.67% was higher than that of Group A and Group B, which was 62.5%（15/24） and 79.17%（19/24） respectively. The total effective rate of Group C 〉Group B〉 Group A, the comparative differences were statistically significant（P〈0.05）.（2） The statistical analysis showed that before treatment, the observation indexes in three groups did not showed statistically significant difference（P〉0.05）. After a month of therapy, 6 MWT, LVEF of Group C was significantly higher than those in Group A and Group B, and LVEDD, IL-6, TNF alpha was significantly lower than Group A and Group B, and the difference was statistically significant（P〈0.05）. The 6 MWT, LVEF, il-6 and TNF-alpha of Group B was significantly higher than Group A, while LVEDD was lower than Group A, and the difference was statistically significant（P〈0.05）.
