摘 要:目的研究IL-37、IFN-γ和IL-10在活动性肺结核患者血清中的表达水平,分析细胞因子间的相关性,探讨其临床意义。方法选取深圳龙岗中心医院2016年9月—2017年4月诊断为活动性肺结核的患者25例为结核组,同期25例健康人群为对照组,通过酶联免疫吸附法(ELISA)检测结核组治疗前后及对照组血清中IL-37、IFN-γ和IL-10水平,并分析其相关性。结果活动性肺结核患者治疗前血清IL-37、IL-10水平较对照组升高[(308.87±62.52)vs.(144.72±45.26)pg/m L;(400.75±46.89)vs.(62.75±5.85)pg/m L],IFN-γ水平下降[(167.25±12.71)vs.(195.50±13.70)pg/m L],差异有统计学意义(P<0.05)。活动性肺结核患者治疗后血清IL-10水平较对照组仍有升高[(120.25±17.04)vs.(62.75±5.85)pg/m L],且差异有统计学意义(P<0.05),IL-37及IFN-γ水平较对照组差异无统计学意义(P>0.05)。活动性肺结核患者治疗前与治疗后相比,IL-37、IL-10水平上升[(308.87±62.52)vs.(137.88±62.47)pg/m L;(400.75±46.89)vs.(120.25±17.04)pg/m L],IFN-γ水平下降[(167.25±12.71)vs.(195.75±4.99)pg/m L],差异有统计学意义(P<0.05)。相关性分析提示IL-37与IFN-γ具有负相关关系。结论血清IL-37、IFN-γ和IL-10水平呈现与活动性肺结核病情相关的动态变化,IL-37对抗结核免疫的负性调节可能与抑制IFN-γ分泌有关。Objective To study the expression level of IL-37,IFN-γ and IL-10 in peripheral blood of active pulmonary tuberculosis patients, analyze the correlation between cytokines and explore its clinical significance. Methods Select 25 patients with diagnosis of active tuberculosis in Longgang District Center Hospital of Shenzhen between September 2016 and April 2017 for TB group, while 25 cases of healthy people as control group at the same time. Use enzyme-linked immunosorbent (ELISA) to detect the levels of serum IL-37, IFN-γ and IL-10 in control group and TB group,including before and after the treatment,analyze their correlation. Results The serum IL-37 and IL-10 levels were higher in the patients with active pulmonary tuberculosis than in the control group [(308.87 ±62.52)vs.(144.72±45.26)pg/mL; (400.75 ±46.89) vs.(62.75 ± 5.85) pg/mL], and the IFN-γ level decreased [(167.25±12.71) vs.(195.50±13.70)pg/mL], and the difference was significant (P〈 0.05). The serum IL-10 level in patients with active pulmonary tuberculosis after the treatment was still higher than that in the control group [(120.25 ± 17.04) vs.(62.75 ± 5.85)pg/mL],and had significant significance (P〈0.05),however there was no significant difference in IL-37 and IFN-γ levels compared with the control group (P〉0.05). Compared with before and post- treatment of active pulmonary tuberculosis patients, IL-37 and IL-10 level increased[(308.87±62.52)vs. (137.88±62.47)pg/mL; (400.75 ± 46.89)vs .(120.25 ± 17.04)pg/mL],and the level of IFN- γ decreased[(167.25 ± 12.71)vs. (195.75 ± 4.99)pg/mL],and the difference was significant (P〈0.05). Correlation analysis indicated that IL-37 had a significantly negative correlation with IFN-γ. Conclusion The levels of serum IL- 37, IFN- γ and IL- 10 showed dynamic changes related to active pulmonary tuberculosis.
