摘 要:目的探讨老年人2型糖尿病患病的危险因素,为老年人2型糖尿病的防治提供科学依据。方法于2017年6—10月采用病例对照的研究方法,从湖南省人民医院内分泌科选取100例老年2型糖尿病患者作为病例组,同院内选取100例老年非糖尿病患者作为对照组,进行问卷调查、体格检查及实验室检测。收集调查对象的指标包括性别、年龄、文化程度、吸烟情况、饮酒情况、身高、体重、体质指数(BMI)、同型半胱氨酸(HCY)、谷丙转氨酶(ALT)、尿素氮(BUN)、肌酐(Cr)、尿酸(UA)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)及餐后血糖(PBG)。结果病例组男性51例,女性49例,平均年龄(74.15±6.926)岁;对照组男性50例,女性50例,平均年龄(75.20±7.862)岁。单因素分析显示,2型糖尿病组吸烟指数、饮酒率、HCY水平与非2型糖尿病组比较,差异有统计学意义(χ~2=9.804,P<0.05;χ~2=14.465,P<0.05;χ~2=12.645,P<0.05);两组其余各指标比较,差异无统计学意义(P>0.05)。多因素非条件logistic回归分析结果显示,饮酒(OR=11.247,P<0.05)、BUN水平较高(OR=2.526,P<0.05)、TG水平较高(OR=2.479,P<0.05)和HCY水平较高(OR=2.721,P<0.05)是老年人2型糖尿病患病的危险因素。结论饮酒、BUN水平、TG水平和HCY水平是老年人2型糖尿病患病的主要影响因素。Objective To study the risk factors of senile patients with type 2 diabetes and provide scientific basis for the prevention and control of senile patients with type 2 diabetes. Methods 100 cases of senile patients with type 2 diabetes admitted and treated in our hospital from June 2017 to October 2017 were selected as the case group, while 100 cases of senile non-diabetic patients were selected as the control group, and the patients were surveyed by the questionnaire, physical examination and laboratory test, and the indexes such as gender, age, culture degree, smoking,drinking, height, weigth, BMI, HCY, ALT, BUN, Cr, UA, TC, TG, HDL-C, LDL-C, FBG, HbA1c and PBG were surveyed. Results In the case group, there were 51 males, and 49 females, the average age was(74.15±6.926) years old,and in the control group, there were 50 males and 50 females, and the average age was(75.20 ±7.862) years old, and the single-factor analysis showed that the differences in the smoking index, drinking rate, HCY level between the type 2 diabetic group and non-type 2 diabetic group, and the difference was statistically significant(χ2=9.804, P 〈0.05;χ2=14.465, P 〈0.05; χ2=12.645, P 〈0.05), and the differences in other indexes between the two groups were not statistically significant,P〈 0.05, and the multi-factor unconditional logistic regression analysis results showed that the smoking(OR=11.247, P〈 0.05), BUN level(OR=2.526, P〈 0.05),TG level(OR=2.479, P〈 0.05) and HCY level( OR=2.721, P〈 0.05)were the risk factors of senile patients with type 2 diabetes. Conclusion Smoking, BUN level, TG level and HCY level are the major influencing factors of senile patients with type 2 diabetes.
