摘 要:目的探讨早期糖尿病肾损伤患者同型半胱氨酸(homocysteine,Hcy)、糖化血红蛋白(hemoglobin A1c,Hb A1c)、尿微量白蛋白(urine microalbumin,U-m Alb)的检测价值。方法比较100例2型糖尿病(type 2 diabetes mellitus,T2DM)患者(观察组,尿蛋白阳性46例和尿蛋白阴性54例)和50例健康志愿者(对照组)Hcy、Hb A1c、U-m Al b检测结果,分析不同尿蛋白检查结果患者Hb A1c、Hcy、U-m Alb单项、两项和三项联合检测异常检出情况。结果观察组患者治疗后FBG、2 h PBG、Hb A1c、Hcy、U-m Alb水平均明显低于同组治疗前,差异有统计学意义(P<0.05);观察组患者治疗前、后FBG、2 h PBG、Hb A1c、Hcy、U-m Alb水平均明显高于对照组,差异有统计学意义(P<0.05);治疗前尿蛋白阳性组患者FBG、2 h PBG、Hb A1c、Hcy、U-m Alb水平>尿蛋白阴性组>对照组,差异有统计学意义(P<0.05);尿蛋白阳性组患者Hb A1c异常、Hcy异常、U-m Alb异常检出率均明显高于尿蛋白阴性组,差异有统计学意义(P<0.05);尿蛋白阳性组患者两项和三项联合检测异常检出率均为100%;尿蛋白阴性组患者两项和三项联合检测异常检出率均明显高于Hb A1c、Hcy、U-m Alb单项检测,差异有统计学意义(P<0.05),其中三项联合检测异常检出率最高,为72.22%。结论 Hcy、Hb A1c、U-m Alb检测能够发现早期糖尿病肾损伤,联合检测能够提高早期糖尿病肾损伤检出率。Objective To explore the detection values of homocysteine( Hcy),glycosylated hemoglobin Alc( Hb A1 c) and urine microalbumin( U-m Alb) in the patients with early diabetic kidney injury. Methods One hundred patients with type 2 diabetes mellitus were enrolled as observe group,in whom,urine protein was positive in 46 cases and urine protein was negative in 54 cases,moreover 50 healthy volunteers were enrolled as control group. The detection results of Hcy,Hb A1 c and U m Alb were analyzed and comapred among the three groups,and analyzed the abnormal detection situation that single detection,the combination detection of two indexes or three indexes in patients with different urine protein detection results. Results After treatment the levels of fasting blood glucose( FBG),2 h postprandial blood glucose( 2 h PBG),Hb A1 c,Hcy,U-m Alb in observation group were significantly lower than those before treatment( P〈 0. 05). The levels of FBG,2 h PBG,Hb A1 c,Hcy,U-m Alb in observation group were significantly higher than those in control group before and after treatment( P〈 0. 05). The levels of FBG,2 h PBG,Hb A1 c,Hcy,U-m Alb in patients with positive urine protein were significantly higher than those in patients with positive urine protein( P〈 0. 05),which in the latter were significantly higher than those in control group( P〈 0. 05). The abnormal detection rates of Hb A1 c,Hcy,U-m Alb in patients with positive urine protein were significantly higher than those of patients with negative urine protein( P〈 0. 05). The abnormal detection rate of combination detection of two indexes and three indexes in positive urine protein group was 100%. The abnormal detection rates of combination detection of two indexes and three indexes were significantly higher than that by single detection of Hb A1 c,Hcy,U-m Alb,respectively( P〈 0. 05). The abnormal detection rate( 72. 22%) of combination detction of three indexes was the highest. Conclusion The detection of Hcy,Hb A1 c
