综合护理干预措施对老年肺癌放化疗患者心理状态及生活质量的影响

摘　　要：目的探讨综合护理干预措施对老年肺癌放化疗患者焦虑和抑郁程度、癌痛、癌性疲乏及生活质量的影响。方法选取2015年6月至2017年12月就诊并接受放化疗的老年肺癌患者120为研究对象,随机分为对照组和观察组,每组60例。对照组采用常规护理措施,观察组在常规护理基础上采用综合性护理干预,比较2组焦虑、抑郁、癌痛和癌性疲乏改善程度和生活质量。结果 2组护理前HAMA、HAMD、VAS及生活质量评分比较差异无统计学意义(P>0.05);2组护理后HAMA、HAMD、VAS评分均较对照组明显降低,生活质量评分均较对照组明显升高,差异有统计学意义(P<0.05),且观察组护理后以上指标改善程度优于对照组(P<0.05)。护理后,观察组癌痛缓解率明显高于对照组,癌性疲乏改善情况明显优于对照组,差异有统计学意义(P<0.05)。结论老年肺癌放化疗患者采用综合护理干预措施不仅能够有效改善患者焦虑、抑郁情绪,缓解患者癌痛、癌性疲乏程度,还能提高患者生活质量,值得临床推广。Objective To observe the effects of comprehensive nursing intervention on mental status and life quality of elderly patients with lung cancer undergoing chemoradiotherapy. Methods A total of 120 elderly patients with lung cancer who were diagnosed and treated by chemoradiotherapy in our hospital from June 2015 to December 2017 were randomly divided into observation group（ n = 60） and control group（ n = 60）. The patients in control group were treated by routine nursing intervention,however,the patients in observation group,on the basis of control group,were treated by comprehensive nursing intervention. After treatment the improvement degree of anxiety,depression,cancer pain and fatigue,as well as life quality of patients were observed and compared between two groups. Results Before nursing,there were no significant differences in HAMA,HAMD,VAS and life quality scores between two groups（ P〉 0. 05）. After nursing,HAMA,HAMD and VAS scores in observation group were significantly lower than those in control group,and life quality scores in observation group were significantly higher than those in control group（ P〈 0. 05）. Moreover,the improve degree of these indexes,the remission rate of cancer pain and the improvement status of fatigue in observation group were superiot to those in control group（ P〈 0. 05）.Conclusion Comprehensive nursing used in the elderly patients with lung cancer after radiation and chemotherapy can not only improve the anxiety,depression emotion,relieve cancer pain of patients,but also can improve their life quality,therefor,which is worthy of clinical promotion.
