蚕食清创换药法联合局部喷洒胰岛素对糖尿病足溃疡面修复愈合的影响

摘　　要：目的观察蚕食清创换药法联合局部喷洒胰岛素对糖尿病足溃疡面修复愈合的影响。方法选取2016年8月至2017年8月期间我院收治的40例糖尿病足患者作为研究对象,随机分为对照组和观察组各20例。对照组患者采用常规清创方法进行治疗,观察组患者采用蚕食清创换药法联合局部喷洒胰岛素进行治疗,对比分析两组患者的治疗效果。结果 (1)观察组患者的治疗总有效率为95.0%,显著高于对照组的70.0%,差异有统计学意义(P<0.05)。(2)治疗后观察组患者的血糖水平显著优于对照组,差异有统计学意义(P<0.05)。(3)观察组患者的创面愈合时间显著短于对照组,差异有统计学意义(P<0.05)。结论蚕食清创换药法联合局部喷洒胰岛素治疗糖尿病足溃疡的效果显著,可降低血糖水平,缩短创面愈合时间,是治疗糖尿病足溃疡的首选方式。Objective To observe the effect of gnawing-away debridement method combined with local insulin spraying on the recovery and healing of diabetic foot ulcer surface. Methods 40 cases of patients with diabetic foot admitted to our hospital from August2016 to August 2017 were selected as research objects. All cases were randomly divided into the control group and the observation group,with 20 cases in each group. The patients in the control group were treated with conventional debridement method, and the patients in the observation group were treated with gnawing-away debridement method combined with local insulin spraying. The curative effects of the two groups were compared and analyzed. Results（1）The total effective rate of the observation group was 95.0%, significantly higher than70.0% of the control group, with significant difference between the two groups（P〈0.05）.（2）After treatment, the blood glucose level of the observation group was significantly better than that of the control group, with significant difference between the two groups（P〈0.05）.（3）The wound healing time of the observation group was significantly shorter than that of the control group, with significant difference between the two groups（P〈0.05）. Conclusions Gnawing-away debridement method combined with local insulin spraying has significant effect in the treatment of patients with diabetic foot ulcer, which can reduce the blood glucose level, shorten the wound healing time, and is the first choice for the treatment of diabetic foot ulcer.
