摘  要:目的 :通过对比SWI与TOF-MRA检出血管内血栓的能力,探讨SWI对急性心源性脑栓塞患者血栓的诊断价值。方法:对发病3 d内的大脑前循环区心源性脑栓塞患者行SWI和TOF-MRA检查。比较SWI检测到的磁敏感血管征象(SVS)与MRA检测的血管闭塞在部位和频率方面的差异。结果 :122例中92例(75.4%)SWI清晰显示SVS,70例(57.4%)MRA显示血管狭窄或闭塞。70例MRA阳性患者显示的闭塞位置与SVS相匹配。52例MRA未检出血管阻塞中22例(42.3%)SWI显示SVS(P<0.001),部位主要在大脑中动脉后分支,其中M2段4例、M3段10例、M4段4例、A3段1例、多部位3例。M1段SVS平均长度13.65 mm(中位数12.39 mm,范围2.70~39.50 mm)。结论:SWI可为急性心源性脑栓塞患者的血栓定位及血栓负荷评估提供有效信息。Objective:Through this study,to compare the detection ability of magnetic susceptibility weighted imaging(SWI)and time-of-flight magnetic resonance angiography(TOF-MRA) in intravascular thrombus,to discuss the value of SWI for diagnosing thrombosis in patients with acute cardiogenic cerebral emboli. Methods:Exams were performed on the patients that with cardiogenic cerebral embolism in anterior cerebral circulation area using SWI and TOF-MRA within 3 days of onset. The differences of SWI detection of magnetic sensitive vascular sign(SVS) and MRA detection of vascular occlusion in position and frequency were compared. Results:Of the 122 patients in the study,92 cases(75.4%) showed a clear SVS on the SWI,and 70 cases(57.4%) showed stenosis or occlusion in the MRA. The 70 cases of MRA positive patients,MRA showed the occlusion position matching with SVS. The 52 cases of MRA were not detected in patients with vascular obstruction,but in those patients,22 cases(42.3%) on the SWI displayed SVS(P〈0.001),mainly in the posterior part of the middle cerebral artery:M2 segment in4 cases,M3 segment in 10 cases,M4 segment in 4 cases,A3 segment in 1 case,multiple parts in 2 cases. The average length of M1 segment SVS was 13.65 mm(median length 12.39 mm,length range 2.70~39.50 mm). Conclusions:SWI provides useful information for the location of thrombosis in patients with acute cardiogenic cerebral embolism and thrombosis load assessment.
