阵发性室上性心动过速患者心房颤动的诱发及其特点

摘　　要：目的探讨阵发性室上性心动过速患者心房颤动的诱发及特点。方法选择无器质性心脏病的阵发性室上性心动过速患者65例,男29例,女36例,年龄18～65(42.6±12.7)岁。在射频消融术成功后予以右心房刺激诱发心房颤动。根据心房颤动诱发成功与否分为房颤组和非房颤组;房颤组根据诱发出的心房颤动持续时间分为<10s、10s～<1min及≥1min3个亚组。比较各组资料和诱发心房颤动的刺激次数与心房颤动诱发成功率以及心房颤动持续时间的关系。结果房颤组37例(56.9%),非房颤组28例(43.1%)。<10s亚组19例(29.2%),10s～<1min亚组8例(12.3%),≥1min亚组10例(15.4%)。所有诱发的心房颤动均自行终止。<10s亚组平均刺激次数为(4.3±2.2)次,10s～<1min亚组为(8.0±3.1)次,≥1min亚组为(10.1±5.0)次。两两比较差异均有统计学意义(t=0.002、0.0002,均P<0.05)。心房颤动的持续时间与左心房前后径大小相关。结论随着心房刺激的次数增加,心房颤动的诱发率增加和持续时间延长。Objective To investigate the induction of atrial fibrillation（AF） and its features in patients without history of AF. Methods 65 patients （29 male, 36 female, 18-65 years old） with paroxysmal supraventricular tachycardia and no structural heart disease were enrolled. After successful ablation, they all underwent right atrial pacing to induce AF and then were divided into AF group and non-AF group according to whether AF was induced. Based on persistent time of AF, AF group was divided into three subgroups （〈10s,10 s-〈1 min and ≥1min=. The clinical data were compared between groups. The relation between atrial pacing stimulation number inducing AF and AF persistent time was analyzed. Results AF was induced successfully in 37（56.9%） patients（AF group） , of them, 19 were in〈10s subgroup, 8 in 10s-〈1 min subgroup, 10 in ≥1min subgroup, and not induced in 28 （43.1%） patients （non-AF group）. All the induced AF terminated spontaneously. The average atrial pacing stimulation was （4.3±2.2）time, （8.0±3.1）time and （10.1±5.0）time in 〈10s subgroup; 10 s-〈1 min subgroup and ≥1min subgroup, respectively. There was significant difference of atrial pacing stimulation time between any two subgroups. The persistent time of induced AF was related to left atrial anteroposterior diameter. Conclusion As atrial pacing stimulation time increasing, the inducibility and persistent time of AF increase.
