钢丝PFNA内固定治疗股骨粗隆间粉碎骨折的临床研究

摘　　要：目的:观察钢丝环扎联合近端抗旋髓内钉(PFNA)内固定治疗股骨粗隆间粉碎骨折的临床疗效。方法:选取2015年2月-2017年2月本院骨科住院行手术治疗的60例股骨粗隆间粉碎骨折患者作为研究对象,根据手术方法的不同分为观察组与对照组,每组30例。对照组仅采用PFNA治疗,观察组采用钢丝环扎联合PFNA治疗。比较两组手术相关指标、术后12个月髋关节Harris功能评分变化及并发症发生情况。结果:两组手术时间、术中出血量、切口长度、术后住院时间及术后愈合时间等手术相关指标比较,差异均无统计学意义(P>0.05)。观察组术后髋关节Harris功能优良率(86.7%)明显高于对照组(60.0%),差异有统计学意义(P<0.05)。观察组并发症发生率(10.0%)明显低于对照组(36.7%),差异有统计学意义(P<0.05)。结论:钢丝环扎联合PFNA内固定治疗股骨粗隆间粉碎骨折可明显改善髋关节功能,显著性降低术后并发症发生率。Objective：To observe the clinical efficacy of steel wire ligation combined with proximal femoral nail antirotation（PFNA）internal fixation in treatment of femoral intertrochanteric comminuted fracture.Method：A total of 60 patients with femoral intertrochanteric comminuted fracture in our hospital from February 2015 to February 2017 were selected and they were divided into control group and observation group according to the operation method,30 cases in each group.The control group was treated with only PFNA,and the observation group was treated with steel wire ligation combined with PFNA.The operation indicators,and the Harris function score changes of the hip joint 12 months after the operation and the complications were compared between the two groups.Result：There were no significant differences about the operative time,the amount of bleeding,the length of the incision, the length of incision,the time of hospitalization and the healing time between the two groups（P〈0.05）.The excellent rate of hip joint Harris function in the observation group was 86.7%,which was significantly higher 60.0% that in the control group,the difference was statistically significant（P〈0.05）.The incidence of postoperative complications in the observation group was 10.0%,which was significantly lower than 36.7% in the control group,the difference was statistically significant（P〈0.05）.Conclusion：The treatment of steel wire ligation combined with PFNA internal fixation for femoral intertrochanteric comminuted fracture can significantly improve the function of the hip joint,and significantly reduce the incidence of postoperative complications.
