术前留置双-J管时间对输尿管软镜碎石术治疗效果的影响

摘　　要：目的评价软性输尿管镜(fURS)前是否留置双-J管及留置时间长短对fURS手术效果的影响。方法回顾性的分析2014年11月至2017年4月新疆医科大学第一附属医院309例接受fURS的结石患者临床资料。将患者按照术前留置双-J管时间分为5组,对5组患者术后并发症,术后无石率,手术时间等13个变量进行统计分析。结果 13个变量单因素分析结果显示,术后并发症、术后无石率、手术时间等5个因素差异具有统计学意义(P<0.05);将有统计学意义的5个变量进一步拟合Logistic回归分析,其中术后无石率(P=0.009;OR=0.06)、术前血象(P=0.001;OR=3.50)、手术时间(P=0.011;OR=0.99)等3个因素有统计学差异(P<0.05)。结论 fURS前预先留置双-J管>7~14d,可缩短手术时间,提高无石率,减少术中及术后并发症的发生,手术更安全。Objective To assess the impact of preoperative ureteral stenting time on the outcome of flexible ureteroscopic lithotripsy（fURS）.Methods The clinical data of 309 patients treated with fURS during Nov.2014 and April 2017 were retrospectively analyzed.According to preoperative indwelling time of double-J stent,the patients were divided into five groups.The operation time,complications,stone free rate,and other variables were statistically analyzed.Results Univariate analysis showed significant differences in preoperative hemogram,operation time,postoperative complications,stone free rate,and hospitalization costs（P〈0.05）.Logistic regression analysis showed significant differences in stone free rate（P=0.009,OR=0.06）,preoperative hemogram（P〈0.001,OR=3.50）,and operation time（P=0.011,OR=0.99）（P〈0.05）.ConclusionIndwelling double-J stent before fURS for 7~14 days can shorten the operation time,improve the stone free rate,reduce complications,and make the operation safer.
