双侧腮腺的唾液腺超声评分系统对原发性干燥综合征的诊断价值

摘　　要：目的单纯测量双侧腮腺的唾液腺超声评分系统对原发性干燥综合征的诊断价值。方法选择原发性干燥综合征患者134例为病例组,同期有口干、眼干症状的非干燥综合征患者109例为对照组。分别依据0~16分评分系统,0~3分评分系统及单纯测量双侧腮腺、颌下腺评分系统对2组受试者进行唾液腺超声检查。结果病例组四种评分系统与对照组比较差异均有统计学意义(P<0.01)。腮腺评分系统的ROC曲线下面积与0~16分、0~3分评分系统均无差异(P>0.05),最佳临界值为5,此时的敏感性为90.30%、特异性86.2%。结论唾液腺超声是诊断原发性干燥综合征敏感而特异的手段。单纯测量双侧腮腺的评分系统简洁而准确。Objective To evaluate the diagnostic value of bilateral parotid salivary gland ultrasound scoring system for primary Sjogren＇s syndrome（ p SS）. Methods A total of 134 patients with PSS were served as case group,at the same time,the other 109 patients with symptoms of dry mouth and dryness of eye without PSS were served as control group. The salivary gland ultrasound（ SGUS） examination was performed according to the four scoring systems including 0 - 16 points SGUS,0 - 3 points SGUS,parotid glands scoring system and submandibular gland scoring system. Results There were significant differences in the average scores by the four scoring systems between two groups（ P〈0. 01）. However there were no significant differences in the area under ROC curve between parotid glands scoring system and 0 - 16 points SGUS,0 - 3 points SGUS scoring system（ P〉0. 05）. The optimal critical value was 5,with sensitivity and specificity being 90. 30% and86. 2%,respectively. Conclusion The salivary gland ultrasound is a sensitive and specific method in diagnosis of PSS,moreover,the simple measurement bilateral parotid gland scoring system may be a simple and accurate way to diagnose PSS.
