门诊2型糖尿病患者药物使用情况调查分析

摘　　要：目的评估某院2016年门诊2型糖尿病(T2DM)患者药物治疗状况及血糖达标情况。方法采用回顾性调查方法,随机抽取某院2016年门诊符合纳入标准的T2DM患者,调查其基本特征、患病情况、治疗方案、血糖控制情况,应用SPSS 22.0统计软件进行统计分析。结果共调查418例T2DM患者。用药频度居于前三的分别是磺脲类、双胍类、DDP-4抑制剂,单个药品以二甲双胍使用最广。糖化血红蛋白(HbA1c)、空腹血糖(FPG)、餐后2 h血糖(2 h PG)平均达标率分别为46.74%、33.33%、22.22%。不同的方案对HbA1c的达标情况差异有统计学意义(P<0.05)。结论某院T2DM患者HbA1c达标率较高,这可能与新型药物DDP-4抑制剂的广泛使用以及联合用药强化治疗有关,临床用药基本合理。T2DM治疗的选择必须根据患者和药物本身特有的属性,以提高整体血糖达标率。Objective To assess the medication status and the condition of blood glucose controlling in outpatients with type 2 diabetes mellitus（ T2 DM） in a hospital in 2016. Methods By retrospective survey method,the outpatients with T2 DM in a hospital in 2016 who conformed to the standard were enroolled in the investigation. The basic characteristics,disease situation,treatment scheme and blood glucose controlling status were statistically analyzed by means of SPSS 22 statistical software. Results A total of 418 patients with T2 DM were investigated. The first three drugs which were used most frequently were sulfonylurea,biguanide,ddp-4 inhibitors,and metformin was most widely used as a single drug. The standard-reaching rates of HbA1c,FPG and 2 h PG were 46. 74% 33. 33% and 22. 22%,respectively. There was a significant difference in standard-reaching rate of HbA1c among different treatment schemes（ P 0. 05）. Conclusion The higher standard-reaching rate of HbA1c in patients with T2 DM in a hospital may be related to the extensive use of new drug ddp-4 inhibitor and the combination of intensive medication,and the clinical medication is basically reasonable. The choice of T2 DM treatment must be based on the specific properties of patients and drugs so as to improve the total standard-reaching rate of blood glucose.
