摘 要:目的分析正常听力婴幼儿chirp-ABR检测反应参数,探索建立chirp-ABR检测各项反应参数的正常参考值范围。方法选择40例(80耳)听力正常婴幼儿,分为A组(3月龄)、B组(6月龄)、C组(1周岁)、D组(3周岁)各10例(20耳)。分别采用CE-chirp及click刺激声进行ABR检测,观察ABR波形辨认率,测出Ⅴ波反应阈,获得不同刺激声强度下Ⅴ波的振幅及Ⅲ波、Ⅴ波潜伏期;对所得检测结果进行统计学分析。结果 (1)当使用80分贝nHL的辨认率刺激强度时,对于chirp-ABRⅠ、Ⅲ、Ⅴ波的辨认率分别为36%、94%、100%,click-ABRⅠ、Ⅲ、Ⅴ波波形辨认率均为100%;(2)各年龄组click-ABR Ⅴ波反应阈值均高于chirp-ABR,但统计学检验结果显示差异无统计学意义,阈值均小于30dB nHL,属于正常范围);(3)在不同刺激声强度下chirp-ABR检测Ⅴ波振幅均高于click-ABR检测Ⅴ波振幅,差异有统计学意义(P<0.05);(4)同一年龄组,相同刺激声强度下,chirp-ABR检测Ⅲ波、Ⅴ波潜伏期较click-ABR短,差异有统计学意义(P<0.05)。结论 chirp-ABR检测具有Ⅰ波辨认率低,Ⅲ波、Ⅴ波潜伏期短,Ⅴ波振幅大,Ⅴ波反应阈值与click-ABR相关性好等特点,临床应用于婴幼儿听力检测应注意这些特点。Objective To analyze the response parameters of chirp-ABR in normal hearing infants, and to explore the normal reference range for the establishment of chirp-ABR to detect various response parameters. Methods 40 cases (80 ears) of normal hearing infants were selected, and each 10 cases (20 ears) were divided into group A (3 months), group B (6 months), group C (1 year old) and group D (3 years old). CE-chirp and click stimulation were used respectively to detect ABR, the ABR waveform identification rate was observed, measure Ⅴ wave response threshold was detected, the amplitude of Ⅴ wave under different exciting acoustic intensity and the latent period of Ⅲ wave, Ⅴ wave were obtained. The results were statistically analyzed. Results (1)When using 80 decibels nHL identification rate of the stimulation intensity, the chirp-ABR Ⅰ, Ⅲ, Ⅴ wave identification rate was 36%, 94%, 100% respectively, the click ABR Ⅰ, Ⅲ, Ⅴwave form identification rate was 100%. (2)The response threshold of click-ABR Ⅴ wave in all age groups was higher than that of chirp-ABR, but the statistical test results showed no statistically significant difference, and the threshold value was less than 30dB nHL, which was in the normal range. (3)The amplitude of V wave detected by chirp- ABR under different exciting acoustic intensity were higher than those detected by click-ABR, and the difference was statistically significant(P 〈0.05). (4) The latent period of Ⅲ wave, Ⅴ wave detected by chirp- ABR under the same age group and the same exciting acoustic intensity were shorter than those detected by click-ABR, and the difference was statistically significant(P 〈0.05). Conclusion Chirp-ABR detection has the characteristics of low identification rate of Ⅰ wave, short latent period of Ⅲ wave and Ⅴ wave, large amplitude of V wave, and good correlation between Ⅴ wave response threshold and click-ABR.
