D-二聚体测定在兔深二度重度烫伤后并发急性肺栓塞诊断中的指导意义

摘　　要：目的:通过检测血浆D-二聚体在深二度重度兔烫伤后的变化,以及深二度重度兔烫伤后不同时期合并肺栓塞时血浆D-二聚体的变化,探讨测定D-二聚体对深二度重度烫伤后合并急性肺栓塞的诊断的指导意义。方法:健康中国大耳白兔48只,雌雄不拘,随机分为:分为对照组、烫伤组、烫伤休克期组、烫伤休克期+肺栓塞组、烫伤感染期组、烫伤感染期+肺栓塞组,每组8只。烫伤组与对照组分别于造模后1 d、3 d、5 d、7 d、14 d、21 d采血检测D-二聚体,其他组于造模后30 min、1 h、3 h、6 h、24 h采血应用酶联免疫法检测D-二聚体。结果:烫伤组与对照组相比,D-二聚体1 d、3 d、5 d、7 d表达高于对照组(P<0.05);烫伤休克期+肺栓塞组与烫伤休克期组相比,D-二聚体的表达差异有显著性(P<0.05);烫伤感染期+肺栓塞组与烫伤感染期组相比,D-二聚体的表达差异亦有显著性(P<0.05)。结论:D-二聚体的表达在各组间都有差异性,D-二聚体可作深二度重度烫伤合并肺栓塞的诊断指标之一。Objective：To detect the changes of plasma D-dimer in severely burnt rabbits on deep partial thickness wound and in different phases with pulmonary embolism, so as cance of D-dimer in the diagnosis of acute pulmonary embolism on deep partial thickness wound.Methods： 48 healthy Chinese white rabbits,male or female,were randomly divided into：control group, bum group,bum shock group,bum shock＋pulmonary embolism group,scald infection phas^ group and bum infection ＋pulmonary embolism group,with 8 rabbits in each group .By mer were determion on l d ^ dJdJdtMd in the burn group and the control group ing built and in 30 min, l h ,3h,6h,24h in the other groups after the model being built. Resmer expression in the burn group were higher than in the control group on obvious difference of expression of D-dimer in the burn shock group and the burn shock ＋ pulmonaryembolism group.＂There were the same obvious difference of expression of D-dimer in the scald infection phase group and bum infection ＋pulmonary embolism group. Conclusion：There were no differences of the expression of D-dimer in each group.＂The D-dimer can be used to predictmonary embolism on the deep partial thickness severe bum wound.
