摘 要:目的 :分析皮肤镜在儿童皮肤真菌感染诊断中的价值,总结不同类型真菌感染的图像特征。方法 :使用皮肤镜观察295例初诊皮肤真菌感染患儿典型皮损,与直接镜检结果进行对比,分析两种方式诊断儿童皮肤感染的准确率,总结皮肤真菌感染的皮肤镜图像特征。结果 :皮肤镜诊断儿童皮肤真菌感染的阳性率为63.05%(186/295),低于直接镜检的83.05%(245/295),差异有统计学意义(P<0.05)。皮肤镜和直接镜检诊断手足体股癣准确率差异无统计学意义,皮肤镜诊断花斑糠疹、甲真菌病、马拉色素毛囊炎、头癣的阳性率均低于直接镜检,皮肤镜阳性病例中,直接镜检阳性者为95.70%,直接镜检阳性病例中,皮肤镜阳性者占71.02%,组间比较差异有统计学意义(P<0.05)。结论 :皮肤镜能够为手足体股癣的临床诊断提供可靠参考,但对于其他类型皮肤真菌感染的诊断价值有限。Objective: The objective of this study was to analyze the value of dermoscopy for diagnosing the skin fungal infections and infestations in children, and to summarize the image features of different types of fungal infections. Methods: Skin lesions of 295 cases of newly diagnosed children patients with skin fungal infections were scanned using dermoscopy. And their results were compared with that scanned by direct microscopy. The accuracy rate of these two methods was compared, and the image features of dermoscopy in detecting the skin infections were summarized. Results: The positive rate of dermoscopy in diagnosing the children skin fungal infection was 63.05%(186/295), which was lower than that of direct microscopy, and the difference was statistically significant(P〈0.05). There was no difference in accuracy rate between dermoscopy and direct microscopy in detecting hand foot and mouth disease. The dermoscopy had lower positive rate in diagnosing pityriasis versicolor, onychomycosis, Marla pigment folliculitis and tinea capitis, as compared to direct microscopy. Among the fungal-positive cases under the dermoscopy, the positive rate was 95.70% under the direct microscopy; while among the fungal-positive cases under the direct microscopy, the positive rate was 71.02% under the dermoscopy, and the difference was statistically significant(P〈0.05). Conclusions: Dermoscopy can provide a reliable reference for hand foot and mouth disease diagnosis, but has limited value in diagnosing other skin fungal infections.
