电针结合归脾汤治疗心脾两虚型失眠的临床研究

摘　　要：目的:观察电针结合归脾汤治疗心脾两虚型失眠的临床疗效。方法:将符合纳入标准的55例患者,随机分为治疗组和对照组。治疗组28例采用电针结合归脾汤治疗,针刺每次30 min,每日1次,归脾汤每日1剂,水煎服。对照组27例单用归脾汤治疗。2周后观察两组患者治疗前后匹兹堡睡眠质量指数(PSQI)评分、5-HT、5-HIAA水平及两组患者治疗后临床疗效的变化情况。结果:PSQI评定方面:两组患者治疗后比较,有显著性差异(P<0.05);治疗组治疗前后比较,有显著性差异(P<0.01),对照组治疗前后比较,有显著性差异(P<0.05)。临床疗效方面:治疗组有效率为92.9%(26/28),对照组有效率为70.4%(19/27),两组临床疗效比较,有显著性差异(P<0.05)。治疗后治疗组5-羟色胺(5-HT)和5-羟吲哚乙酸(5-HIAA)水平均高于对照组(P<0.05);与本组治疗前比较,差异极显著(P<0.01)。结论:电针结合归脾汤治疗心脾两虚型失眠,可以有效改善患者睡眠情况,改善5-HT和5-HIAA水平,且临床疗效肯定,值得进一步临床推广。Objective： To observe the clinical effect of electro - acupuncture combined with Guipi Decoction oninsomnia of deficiency of heart and spleen. Methods： 55 insomnia patients were randomly divided into thetreatment group（n = 28） and the control group（n = 27）. The treatment group was given electro - acupuncturecombined with Guipi Decoction, whereas the control group was only treated with Guipi Decoction. Scores ofPSQI were observed before and after the treatment in the both groups and the clinical efficacy was comparedbetween the two groups after two weeks of the treatment. Results： In terms of the PSQI assessment, there wassignificant difference between the two groups after the treatment（P 〈 0. 05）, the difference was greatly significant between before and after the treatment in the treatment group（P 〈 0. 01）, it was also statistically signifi￣cant in the control group（P 〈 0. 05）. The effective rate was 92. 9% （26 / 28） in the treatment group, whichwas significantly higher than 70. 4% （19 / 27） in the control group（P 〈 0. 05）. Levels of 5 - HT and 5 -HIAA were both higher in the treatment group than those in the control group（P 〈 0. 05）. Conclusion： Thetherapy of electro - acupuncture combined with Guipi Decoction can effectively improve sleep condition and thelevels of 5 - HT and 5 - HIAA in the treatment of insomnia of deficiency of heart and spleen. Its clinical curative effect is positive and worthy of further clinical application.
