摘  要:目的探究对老年中重度慢性阻塞性肺疾病稳定期患者进行慢阻肺评估测试、6 min步行试验和肺功能检测的诊断价值。方法随机选择97例于2016年7月—2017年6月间该院收治的患者,对其进行6 min步行试验、慢阻肺评估测试、肺功能检测。结果Ⅱ级患者FEV1%预计值为(59.06±6.85)、Ⅲ级患者预计值为(42.05±5.39)、Ⅳ级患者为(24.26±5.02),Ⅱ级、Ⅲ级患者FEV1%预计值差异有统计学意义(t=12.874 3,P=0.023 1),Ⅱ级、Ⅳ级患者FEV1%预计值差异有统计学意义(t=21.321 9,P=0.013 8)。Ⅲ级、Ⅳ级患者CAT评分、FEV1%预计值与Ⅱ级患者差异有统计学意义(P<0.05),6MWD差异无统计学意义(P>0.05),Ⅳ级患者CAT评分、FEV1%预计值与Ⅱ级、Ⅲ级患者差异有统计学意义(P<0.05),6MWD对比差异无统计学意义(P>0.05)。CAT分级与中重度慢性阻塞性肺疾病分级、6MWT分级具有较高符合率。结论对老年中重度慢性阻塞性肺疾病稳定期患者进行慢阻肺评估测试、6 min步行试验诊断价值较高。Objective This paper tries to investigate the diagnostic value of chronic obstructive pulmonary disease(COPD)test, 6-minute walk test and pulmonary function test in elderly patients with stable chronic obstructive pulmonary disease(COPD). Methods A total of 97 patients admitted to our hospital from July 2016 to June 2017 were randomly selected. They were subjected to 6-minute walk test, COPD evaluation test and pulmonary function test. Results The predicted FEV1% in grade II patients was(59.06±6.85), the predicted value in grade III patients was(42.05±5.39), and(24.26±5.02) in grade IV patients. The difference in predictive value of FEV1% between grade II and grade III patients was statistically significant(t=12.874 3, P =0.023 1). There was a statistically significant difference in the predicted FEV1% of patients with grade II and IV(t=21.321 9, P=0.013 8). There were significant differences between grade III and IV patients' CAT scores, FEV1%predicted values and grade II patients(P〈0.05). There was no significant difference in 6 MWD(P〉0.05). Grade IV patients had CAT scores and FEV1% predicted values. There was a significant difference between patients with grade II and grade III(P〈0.05), and there was no significant difference between 6 MWD(P〉0.05). There was a high coincidence rate between CAT classification and moderate-severe chronic obstructive pulmonary disease grading and 6 MWT grading. Conclusion Chronic obstructive pulmonary assessment testing and 6-minute walking test in elderly patients with stable chronic obstructive pulmonary disease have higher diagnostic value.
