摘　　要：目的研究探讨小切口PFNA治疗老年骨质疏松股骨粗隆间骨折临床效果。方法方便选择2007年1月—2017年12月该院老年骨质疏松股骨粗隆间骨折患者41例进行研究讨论,把所有患者随机分为两组,对照组20例,观察组21例,其中对照组采用短重建髓内钉固定治疗,观察组采取小切口PFNA治疗,观察两组患者的治疗效果、Harris评分。结果观察组患者的手术时间、术中出血量、骨折愈合时间分别为(55.5±10.5)min、(124.0±51.3)m L、(3.1±1.5)m,均明显优于对照组,差异有统计学意义(t=8.118、4.312、4.008,P<0.05)。观察组中Harris评分为差的患者仅有1例(4.8%),所占比率明显低于对照组的30.0%,两组差异有统计学意义(χ~2=4.448,P<0.05)。结论对于老年骨质疏松股骨粗隆间骨折的患者,采取小切口PFNA进行治疗,效果显著,不会导致患者出现较大的创伤,且术中出血量少,固定较好,可以有效促进患者髋关节的恢复,值得在今后患者的治疗过程中应用。Objective This paper tries to study the clinical effect of small incision PFNA in the treatment of senile osteoporotic intertrochanteric fracture of femur. Methods From January 2007 to December 2017, 41 patients with osteoporotic femoral intertrochanteric fracture in the hospital were convenient selected for study and discussion. All patients were randomly divided into two groups： control group, 20 cases, observation group, 21 cases. A short reconstructive intramedullary nail was used for treatment in the control group. The observation group was treated with small incision PFNA. The treatment effect and Harris score of the two groups were observed. Results The operative time, intraoperative blood loss, and fracture healing time in the observation group were（55.5±10.5）min,（124.0±51.3）m L, and（3.1±1.5）m, respectively, which were significantly better than those in the control group. The difference was statistically significant（t=8.118, 4.312, 4.008, P0.05）.In the observation group, only one patient（4.8%） had a poor Harris score, which was significantly lower than the control group（30.0%）. There was a statistically significant difference between the two groups（χ2=4.448, P 〈0.05）. Conclusion For elderly patients with osteoporotic intertrochanteric fractures, the use of small incision PFNA for treatment has significant results, will not lead to patients with greater trauma, and less intraoperative blood loss, better fixation, can effectively promote the patient＇s hip joint recovery, which is worthy of application in the treatment of patients in the future.
