摘 要:目的分析对CCP抗体采用吖啶酯直接化学发光法和ELISA定量检测的临床作用。方法方便选择2017年1月—2018年1月来该院检验科进行CCP抗检测的272例患者,所有患者均采用吖啶酯直接化学发光法与ELISA定量检测,对各组检测结果进行分析比较。结果采用吖啶酯直接化学发光法的CCP抗体诊断中,特异性为96.69%,敏感性为69.49%,阳性预测值与阴性预测值为87.13%、91.91%。采用ELISA定量检测法的CCP抗体诊断中,特异性为97.06%,敏感性为68.01%,阳性预测值与阴性预测值为92.28%、86.03%;在类风湿性关节炎患者中,活动期CCP抗体含量(141.11±93.96)IU/m L,非活动期CCP抗体含量(45.15±10.56)IU/m L,由此可见类风湿性关节炎活动期CCP抗体含量明显高于非活动期(t=8.369,P=0.000);采用吖啶酯直接化学发光法与ELISA检测的CCP抗体,阴性符合率为100.00%,阳性符合率为94.44%,总符合率为98.53%。结论针对CCP抗体,吖啶酯直接化学发光法与ELISA定量检测具有良好的符合率,吖啶酯直接化学发光法适合在仪器自动化中应用,易标准化,操作简单,与临床要求相符合。CCP抗体作为类风湿性关节炎诊断的一种新血清学标志物,对类风湿性关节炎病情监测有利。Objective This paper tries to analyze the clinical effect of anti-CCP using acridinium ester direct chemiluminescence and ELISA. Methods 272 cases of CCP anti-detection were selected conveniently from January 2017 to January2018 in the laboratory. All patients were tested with acridinium ester direct chemiluminescence and ELISA. The results of each group were analyzed and compared. Results In the diagnosis of CCP antibodies using acridinium ester direct chemiluminescence method, the specificity was 96.69%, the sensitivity was 69.49%, and the positive predictive value and negative predictive value were 87.13% and 91.91%. In the diagnosis of CCP antibodies using ELISA quantitative assay, the specificity was 97.06%, the sensitivity was 68.01%, and the positive predictive value and negative predictive value were 92.28%and 86.03%; in patients with rheumatoid arthritis, active CCP antibodies content(141.11±93.96)IU/m L, inactive CCP antibody content(45.15±10.56) IU/m L, it can be seen that the CCP antibody content of active rheumatoid arthritis was significantly higher than inactive period(t=8.369,P=0.000); CCP antibodies detected by acridinium ester direct chemiluminescence method and ELISA, the negative coincidence rate was 100.00%, the positive coincidence rate was 94.44%, and the overall coincidence rate was 98.53%. Conclusion For CCP antibodies, acridinium ester direct chemiluminescence method and ELISA quantitative detection have a good coincidence rate, acridinium ester direct chemiluminescence method is suitable for application in instrument automation, easy standardization, simple operation, and in line with clinical requirements. As a new serological marker for the diagnosis of rheumatoid arthritis, CCP antibody is beneficial to the monitoring of rheumatoid arthritis.
