神经内窥镜手术对高血压脑出血患者神经功能及抑郁状态的影响

摘　　要：目的探讨神经内窥镜手术对高血压脑出血患者神经功能及抑郁状态的影响。方法将我院收治的258例高血压脑出血的患者根据手术方式分为A组和B组,各129例。A组接受神经内窥镜手术,B组接受小骨窗开颅术。比较两组患者术后并发症、死亡、神经功能缺损及抑郁情况。结果 A组术后并发症总发生率及抑郁发生率明显低于B组(P<0.05)。术后1、3个月,两组抑郁患者的NIHSS评分明显高于非抑郁患者(P<0.05);不同抑郁状态下,A组术后1、3个月NIHSS评分均明显低于B组(P<0.05)。结论相较于小骨窗开颅术,神经内镜术应用于高血压脑出血有明显优势,但是全麻术对基础疾病较多的老年患者有一定局限性,在临床上选择手术时应根据患者的实际情况选择患者适合的手术方式。Objective To investigate the effects of neuroendoscopic surgery on neurological function and depression inpatients with hypertensive cerebral hemorrhage. Methods A total of 258 patients with hypertensive cerebral hemorrhage inour hospital were divided into group A and groupB according to the operation methods, with 129 cases in each group. Thegroup A was given neuroendoscopic surgery, and the groupB was treated with small bone flap craniotomy. The occurrenceof complications, death, neurologic impairment depression were compared between the two groups. Results The totalincidence of postoperative complications and incidence of depression in the group A were lower than those in the groupB（P〈0.05）. At 1 and 3 months after operation, the NIHSS scores of patients with depression in the two groups were higherthan those of patients without depression（P〈0.05）. With different status of depression, the NIHSS scores at 1 and3 monthsafter operation in the group A were lower than those in the groupB （P〈0.05）. Conclusion Compared with small bone flapcraniotomy, neuroendoscopic surgery has obvious advantages in the treatment of hypertensive cerebral hemorrhage.However, general anesthesia has some limitations in older patients with more basic diseases. In clinical practice, theappropriate surgical methods should be selected according to the actual situation of patients.
