摘  要:目的观察呼吸机机械通气的慢性肺源性心脏病患者使用不同剂量地高辛治疗的不良反应发生情况。方法回顾性分析80例呼吸机机械通气的慢性肺源性心脏病患者,按地高辛使用情况分成三组,36例接受0.125 mg地高辛治疗,12例接受0.25mg地高辛治疗,32例非地高辛治疗患者作为对照。三组患者治疗7天后,测定地高辛浓度,患者BNP、UCG的情况,观察地高辛不良反应状况。结果治疗7天后,0.125 mg地高辛组患者与非地高辛组患者相比BNP改善明显,且不良反应发生率低。0.25mg地高辛治疗组患者由于出现明显的心律失常,给予地高辛减量或停用,均未用至7天。结论 0.125 mg地高辛治疗呼吸机机械通气的慢性肺心病患者相对安全,可以改善患者的心功能。Objective To observe the adverse reaction in patients with pulmonary heart disease undergoing mechanical ventilation who received different dose of digoxin. Methods 80 patients with pulmonary heart disease undergoing mechanical ventilation were divided into three groups. 36 cases in the group A received digoxin 0. 125 mg Qd,12 cases in the group B received digoxin 0. 25 mg Qd,and 32 without digoxin treatment as the control group.Their digoxin level,BNP,UCG and adverse reaction of digoxin were observed 7 days after treatment. Results 7 days after treatment,BNP level and adverse reactions improved more obviously in the group A and the control than in the group B( P 0. 05). Because of obvious arrhythmia,the group B had to reduce the digoxin dose or stopped to use it,and they all received digoxin less than 7 days. Conclusion The dose of 0. 125 mg digoxin is relatively safe and can improve their heart function in treatment of patients with pulmonary heart disease undergoing mechanical ventilation.
