无创比例辅助通气与压力支持通气对慢性阻塞性肺疾病的临床价值

摘　　要：目的探讨无创比例辅助通气对慢性阻塞性肺疾病的临床价值。方法选择2016年9月至2017年8月本院收治的慢性阻塞性肺疾病急性发作患者96例,随机数字法分为两组,各48例,对照组行常规压力支持辅助无创通气,观察组实施无创比例辅助通气,比较两组干预后血气分析中氧代谢及肺功能相关指标变化情况,统计两组呼吸机治疗及总住院时间,两组有创气管插管率及救治成功率。结果干预后,观察组氧摄取率高于对照组(P<0.05),动脉血氧含量高于对照组(P<0.05),观察组FEV_1大于对照组(P<0.05),FEV_1/FVC水平高于对照组(P<0.05),观察组呼吸机治疗时间短于对照组(P<0.05),总住院时间短于对照组(P<0.05),观察组有创气管插管率显著低于对照组(P<0.05),救治成功率显著高于对照组(P<0.05)。结论针对慢性阻塞性肺疾病急性发作患者行无创比例辅助通气,更好的改善机体氧供需平衡,促进肺功能恢复,缩短呼吸机治疗时间及住院时间,提高临床救治成功率。Objective To investigate the clinical value of noninvasive proportional ventilation for patients with chronic obstructive pulmonary disease. Methods 96 patients with AECOPD from May 2016 to August 2017 were randomly and evenly divided into two groups. The control group received routine pressure support assisted with noninvasive ventilation,and the observation group was given noninvasive proportional ventilation. The changes of oxygen metabolism and lung function were compared between the two groups. The duration of ventilator and hospital stay was recorded. The tracheal intubation rate and the success rate were analyzed. Results The oxygen uptake rate,the arterial oxygen content and the levels of FEV_1 and FEV_1/FVC were obviously higher in the observation group than in the control group（ P〈0. 05）. The duration of treatment and hospital stay was shorter in the observation group than in the control group（ P〈0. 05）. The invasive tracheal intubation rate was significantly lower in the observation group than in the control group（ P〈0. 05）,and the success rate of treatment was significantly higher in the observation group than in the control group（ P〈0. 05）. Conclusion Noninvasive proportional ventilation can balance oxygen supply and demand,promote the recovery of lung function,shorten the duration of ventilator treatment and hospital stay and improve the success rate of clinical treatment in patients with chronic obstructive pulmonary disease.
