摘　　要：目的评价CO_2点阵激光联合0.1%他克莫司软膏外用治疗斑秃的临床疗效。方法 30例(60处皮损)多发性斑秃患者,随机分为治疗组和对照组,每组15例(30处皮损)。治疗组患者给予CO_2点阵激光联合0.1%他克莫司软膏外用治疗,对照组患者仅给予0.1%他克莫司软膏外用治疗。比较两组患者的治疗效果,观察两组患者不良反应发生情况。结果治疗组患者皮损处治疗痊愈10处,显效13处,有效7处,无效0处;对照组患者皮损处治疗痊愈2处,显效14处,有效8处,无效6处。治疗组患者皮损处治疗效果优于对照组,差异具有统计学意义(U=2.728,P<0.05)。治疗组起效快,新生毛发更粗和更密。CO_2点阵激光治疗即刻有烧灼刺痛感,治疗区轻微红肿,无渗出,24 h后不适感自行消退;使用他克莫司软膏外用头皮未发现明显刺激发红及不能耐受者。两组患者术后未见水疱、感染、色素沉着等明显不良反应。结论 CO_2点阵激光联合0.1%他克莫司软膏外用治疗斑秃的临床疗效显著,安全性高,值得临床选用。Objective To evaluate the clinical efficacy of fractional CO2 laser combined with 0.1% tacrolimus ointment for the treatment of alopecia areata. Methods A total of 30 patients with multiple alopecia areata were randomly divided into treatment group and control group, with 15 cases （30 skin lesions） in each group. The treatment group received fractional CO2 laser combined with 0.1% tacrolimus ointment for external treatment, and the control group received 0.1% tacrolimus ointment for external treatment. The treatment effect in two groups was compared, and the occurrence of adverse reactions between the two groups was observed. Results The treatment group had 10 cured lesions, 13 excellent lesions, 7 effective lesions and 0 ineffective lesion, which were 2, 14, 8 and 6 lesions in the control group. The treatment group had better treatment effect of lesions than the control group, and the difference was statistically significant （U=2.728, P〈0.05）. The treatment group worked fast and the newborn hair was thicker and denser. The CO2 laser treatment immediately had a burning tingling sensation, and the treatment area was slightly red and swollen. There was no exudation, and discomfort subsided after 24 h. Application of tacrolimus ointment on the topical scalp did not lead to significant irritancy to redness and intolerance. No obvious adverse reactions of blisters, infection, and pigmentation were found after operation in both groups. Conclusion Combination of fractional CO2 laser and 0.1% tacrolimus ointment shows remarkable clinical efficacy for the treatment of alopecia areata with high safety, and it is worthy of clinical application.
