摘 要:目的探究神经肌电图在诊断糖尿病早期周围神经病变中的应用价值。方法选取2015年1月至2017年1月在首都医科大学附属北京友谊医院接受治疗的140例不同病程(病程<1年,47例;病程1~10年,55例;病程≥10年,38例)的糖尿病患者作为前瞻性研究对象,对所有患者给予神经肌电图检查,比较不同病程患者正中神经、尺神经、腓浅等运动传导速度(MCV)、感觉传导速度(SCV),并对患者尺神经f波、胫神经H反射情况进行分析。结果病程≥10年组患者腓总神经MCV异常率达到42.1%(16/38),显著高于正中神经MCV、尺神经SCV异常率,腓总神经SCV异常率达到44.7%(17/38),显著高于病程1~10年组、病程<1年组,差异具有统计学意义(P<0.05);糖尿病患者胫神经H反射异常率显著高于尺神经f波异常率,病程≥10年组患者胫神经H反射及尺神经f波异常率分别为92.1%、21.1%,显著高于病程1~10年组、病程<1年组患者,差异具有统计学意义(P<0.05)。结论神经肌电图检查能够有效评估糖尿病患者早期周围神经病变,具有较高的诊断价值。Objective To explore the application value of electromyography in diagnosis of peripheral neuropathy in early diabetic patients. Methods 140 patients with different course of diabetes,who were treated in Beijing Friendship Hospital from January 2015 to January2017,were treated as subjects in this prospective study. All patients were given electromyography. The motor conduction velocity( MCV) and sensory conduction velocity( SCV) of the median nerve,ulnar nerve and superficial peroneal( SCV) were compared in the patients with different course of disease(〈 1 year,47 cases; 1 - 10 years,55 cases; ≥10 years,38 cases),and the ulnar nerve was used in the patients. The F wave and the H reflex of the tibial nerve were analyzed. Results The abnormal rate of MCV in the peroneal nerve was 42. 1%( 16/38),which was significantly higher than that of the median nerve MCV and the ulnar nerve SCV,and the abnormal SCV rate of the peroneal nerve was 44. 7%( 17/38),which was significantly higher than that in the course of disease 1 - 10 and the course of disease 1( P〈0. 05),and the reflex of the tibial nerve H was different in diabetic patients. The rate of abnormal F wave in ulnar nerve was significantly higher than that in group 10. The abnormal rate of H reflex of tibial nerve and the abnormal F wave of ulnar nerve were 92. 1% and 21. 1% respectively,which were significantly higher than those in the course of disease 1 ~ 10 and the course of disease 1 years,the difference was statistically significant( P〈0. 05). Conclusion Electromyographic examination of diabetic peripheral neuropathy in early stage can effectively assess the patient's condition and has high diagnostic value.
