摘　　要：目的分析患者医院门诊不明原因发热(fever of unknown origin,FUO)的相关因素。方法选取2015年6月-2016年6月在医院门诊进行治疗的不明原因发热患者158例,根据其病历资料分为两组,分别为感染组84例和非感染组74例,收集患者在医院门诊部的临床表现、体检结果和实验室研究结果,建立不明原因发热患者的数据库,进行回归分析。结果关节痛、发热病程、血清铁蛋白、体质量减轻、中性粒细胞是明确与感染性疾病相关的因素,且FUO病情与以上各指标呈负相关,根据本次研究建立的模型,参数回归模型的ROC曲线下面积达到0.8578,95%CI在0.8578~0.9126,最佳诊断值是2.32。结论关节痛、病程、体质量减轻、ALP、SF中性粒细胞比例为FUO中与感染性疾病相关的因素,且之间相互作用,值得临床关注。OBJECTIVE To snalyze the related factors for fever of unknown origin（FUO）amony outpatients.METHODS A total of 158 patients with FUO who were treated in outpatient department from Jun 2015 to Jun2016 were enrolled in the study and divided into two groups according to medical records：the infection group with84 cases and the non-infection group with 74 cases.The clinical manifestations and results of physical examination and laboratory examination were collected,the database of the patients with FUO was established,and the regression analysis was performed.RESULTS The joint pain,fever course,serum ferritin,weight loss and neutrophil were the factors related to infectious diseases.FUO was negatively correlated with the above indexes.According to the model and parameters established in this study,the area under the ROC curve of the regression model was 0.8578,the 95%CI was between 0.8578 and 0.9126,and the optimal diagnostic value was 2.32.CONCLUSIONThe joint pain,course of disease,weight loss and ration of ALP to SF are related factors for infectious diseases in the patients with FUO,and the interaction between them should be attached great importance.
